2 WELCOME INVITATION Dear Colleagues, We are pleased to inform you that the sequel to the 2008 Bone Symposium held in Las Vegas will take place in San Francisco on May 18 19, 2012, at the Grand Hyatt Hotel. This year s theme will be From Autogenous Bone to Tissue Engineering: A Paradigm Shift for Intraoral Regenerative Procedures. It is no secret that our treatment options have been enhanced with the continued introduction of new biomaterials and surgical advances that are challenging to traditional treatment paradigms. This symposium is an opportunity to review current advances, reevaluate our cherished beliefs, and take home a significant clinical message. We have gathered authorities in their respective fields to share their knowledge and clinical observations. Presented topics will include periodontal regeneration, horizontal and vertical autogenous ridge augmentation, sinus elevation, and the correction of esthetic deformities. 3 GENERAL INFORMATION REGISTRATION Your registration fee includes admission to all symposium sessions and exhibits, daily breakfast and coffee breaks, Friday lunch, and the symposium reception on Friday evening. Take advantage of specially reduced fees by registering in advance. Deadline for registering for the symposium is May 4, CANCELLATIONS/REFUNDS Cancellations made in writing and received by Quintessence before May 4, 2012, will be refunded in full. For cancellations received on or after May 4, 2012, but before the start of the meeting, a cancellation fee of US $75 will be charged. No refunds will be made after the symposium begins on May 18, CONTINUING EDUCATION CREDITS One hour of continuing education (CE) credit is offered for every hour of program attendance. CE credit awarded for participation in a course may not apply toward licensing renewal in all states. It is the responsibility of each participant to verify the requirements of his or her state licensing board. Meeting attendees are responsible for ensuring that their education credits are current and on file with their respective state board or other licensing/regulatory agency. 4 ABSTRACTS FRIDAY, MAY 18, :00 am 10:00 am Osteogenic Procedures with Recombinant Growth Factors Myron Nevins, dds The introduction of recombinant growth factors into the dental armamentarium has provided a new paradigm of treatment for the discerning clinician. Their use enhances the results of both periodontal regeneration and bone development in implant placement. This presentation will offer proof of principle for the use of recombinant growth factors for the successful treatment of vertical infrabony defects and a variety of localized ridge defects. Topics will include sinus elevation, horizontal and vertical ridge augmentation, and the reconstruction of compromised extraction sites. Preclinical and clinical investigations will be reviewed. 10:00 am 11:00 am Clinical Development of Growth Factor Therapies William Giannobile, dds, dmsc Repair of soft and hard tissue defects caused by periodontal disease or trauma is a major goal of reconstructive therapy. Over the past decade, investigators have begun to use signaling molecules such as platelet-derived growth factor (PDGF), bone morphogenetic proteins (BMPs), and biomimetic peptides to restore lost tooth support due to damage to the alveolar process. This presentation will review emerging clinical therapies in materials science and growth factor biology. In addition, the presentation will highlight the use of polymeric delivery systems and image-based scaffolding technologies that aid in the targeting of proteins, genes, and cells for reconstruction of periodontal and peri-implant defects. The presentation will conclude by addressing the combined approach of regenerative molecule delivery and host modulation therapies to treat periodontal osseous defects. 11:30 am 1:00 pm Clinical Trends in Bone Augmentation: Is Autogenous Bone Still the Gold Standard? Craig Misch, dds, mds Autogenous bone has long been the gold standard of bone graft materials because it provides predictable volume gains and favorable bone quality with short healing periods. However, the need to harvest the graft and the associated morbidity are inherent disadvantages. The choice of a technique for bone augmentation depends on several factors, including ridge morphology, degree of atrophy, bone volume requirements, and clinician preference. Successful bone regeneration may be obtained with numerous techniques. More recently, recombinant human bone morphogenetic protein 2 (rhbmp-2) has been shown to be effective in repairing buccal wall defects from extractions and in sinus bone grafting. The use of rhbmp-2 is currently under clinical investigation as a potential replacement for autograft in ridge augmentation procedures. 2:30 pm 3:30 pm A Novel Approach for Soft Tissue Augmentation David Kim, dds, dmsc The importance of a functional and healthy mucogingival complex to preserve the underlying osseous stability around teeth is evident by the increasing attention given to soft tissue augmentation procedures in clinical practice. An intact attached band of keratinized gingiva is seen as a critical component to the protective function of the mucogingival complex, although the minimum width requirement for this tissue type remains controversial. This presentation evaluates the use of a xenogenic porcine bilayer collagen matrix to augment the zone of keratinized tissue around teeth. 3:30 pm 5:00 pm Regenerative and Esthetic Techniques in Implant Surgery: Clinical Applications with Recombinant Growth Factors Marc Nevins, dmd, mmsc The integration of recombinant growth factor technology into the surgical armamentarium is changing the approach to dental implant site development by reducing the need for autogenous bone grafting and allowing for less-invasive procedures. Use of tissue engineering allows clinicians to achieve optimal biologic and esthetic results for challenging periodontal and implant cases. This presentation will update techniques for esthetic implant site development using recombinant human platelet-derived growth factor BB (rhpdgf-bb). Cases will demonstrate the successful use of combination rhpdgf-bb and bone replacement grafts to treat extraction socket defects and perform ridge augmentation in preparation for implant placement. Techniques for combined hard and soft tissue grafting, surgical management of esthetic challenges, and minimally invasive techniques will also be discussed. SATURDAY, MAY 19, :30 am 10:00 am Alveolar Ridge Regenerative Strategies: Autogenous Bone Versus Tissue Engineering Michael Pikos, dds This clinical presentation will compare the use of autogenous bone and tissue engineering protocols including recombinant human bone morphogenetic protein-2 (rhbmp-2), recombinant human platelet-derived growth factor (rhpdgf), and plasma-rich growth factor (PRGF) for alveolar ridge augmentation. The science, indications, advantages, and disadvantages of each approach will be featured. Single-tooth to full-arch reconstruction cases will be covered. 10:30 am 12:00 pm The Restorative Implications of Regeneration- Enhanced Prosthetic Results Maurice Salama, dmd Progressive clinicians have access to an astounding array of new tools, technology, and materials. However, none of these advances can ensure success if the treatment design is inadequate. This presentation puts an emphasis on identifying the risk factors that can jeopardize successful esthetic outcomes and then developing appropriate solutions to overcome them. A clear blueprint will be outlined that synergistically incorporates digital and biomaterial technology with advanced surgical protocols to overcome esthetic challenges and predictably achieve successful results. In addition, specific guidelines for the use of new regenerative products will be outlined as well as new applications for future enhancement. 12:00 pm 1:30 pm Tissue Engineering and Cleft Palate Repair Michael Carstens, md The current discussion of treatment of cleft palate patients is based on a model of cleft embryology essentially unchanged since the late 19th century. Accurate knowledge of the organization of embryonic tissues enables surgeons to think about cleft lip/palate from the perspective of tissue engineering. This takes three forms: (1) stem cell reassignment, (2) stem cell stimulation, and (3) stem cell implantation. This presentation will discuss the neuroangiosome model of the maxillary complex. The relationship between deficiency states in underlying bone fields and overlying soft tissue fusion will also be described. New surgical concepts of cleft lip/palate repair will be covered in some detail.
