Todd Heckel 5 Discussion Elizabeth Todd Heckel welcomed everyone and commenced the meeting at 9:55 am. 2. Minutes from March 3, 2017 meeting All 3 The minutes from the March 3, 2017 meeting were approved as written. 3. Update Contact Information All 2 Ms. Todd Heckel asked everyone to review their contact information on the sign-in sheet, and make any necessary updates. 4. Wins, Announcements Dr. Jenkins informed the group that she has been awarded a R01 that will be focused on controlling blood pressure for stroke survivors. Half of the participants will have a diagnosis of diabetes. The grant will be between MUSC, Greenville Hospital System and Richland Memorial. All 10 / Discussion Diane Mathews reported on a SC AHEC federal grant for AHEC scholars. The grants focus is to get primary care practitioners into rural areas. LC AHEC was also written into a new grant at MUSC that will provide preceptor training videos for student motivation. Dr. Michelle Nichols reported on a grant on diabetes in Ghana. This will be a nurse led education in diabetes care using mobile technology. She will also be working with AHEC on an intervention study. Eileen Ward reported that the Upper Piedmont Association of Diabetes Educators will be holding a diabetes management conference in Greenville in November. Shayla Jones with DHEC reported that Novo Nordisk and ADA will be
3 Pillar holding a policy summit on December 5 from 10am 2pm in Columbia. The purpose of the summit is to bring experts together to share best practices for policy. There will be legislative participation. Elizabeth Todd Heckel noted that Morgan Adams, PharmD will provide a 2 hour diabetes education program for the legislators and State House staff in October. 5. Diabetes Community Health Programs As previously reported, Dr. Jenkins reviewed the R01 that will be focused on controlling blood pressure for stroke survivors. Half of the participants will have a diagnosis of diabetes. The grant will be between MUSC, Greenville Hospital System and Richland Memorial. 6. Policy and Advocacy Sharon Eubanks of CCME sent an to DSC explaining that The Carolinas Center for Medical Excellence (CCME) has cancelled its DSME program due to lack of additional funding. She noted that the program has already far exceeded CMS initial DSME goal for the state, having educated and graduated more than 1,100 Medicare Beneficiaries with diabetes. 7. DHEC Rhonda Hill reported for DHEC. Their CDC 1305 & 1422 grants will be ending in June 2018 and September 2018, respectively. The next 5-year grant will be released in January The main focus will be on health systems and quality improvement. DAC regularly meets to discuss their 4 pillar goals. Separate groups work on developing each pillar. One of their objectives is to have their own diabetes toolkit that will be based on the one developed by the ADA. The 16 th Annual Chronic Disease Symposium is scheduled for March 9 10, 2018 in Myrtle Beach. It will have an evidence based strategies theme. DHEC will be accepting nominations for four champion awards.
4 Pillar 8. Outreach to Board Elizabeth Todd Heckel noted that Maria McGregor is the new camp mom. Kathy Lewis, MD was very instrumental at camp. They had over 75 new volunteers and over 300 type 1 participants. 9. Vocational Rehabilitation Freda King reported a 5% increase on cases served in the past 3 months. 10. Grants Update Dr. Jenkins noted that Tim Lyons, MD and Lou Luttrell, MD have been awarded a CoBRE grant. Gayenell Magwood, PhD has been awarded a grant subaward for diabetes prevention using the Pittsburgh curriculum. The original award is ending and will be continuing with a larger grant. Michelle Nichols, PhD noted that there is another project in the works that will offer further intervention refinement for those in public housing. John Douglas of the ADA reported that the ADA and Wal-Mart are partnering on a program called What Can I Eat. The program will serve people a year, and focus on participant preferences, cooking, shopping, and diet. If funding is received, there will be one program in the Pee Dee area near Florence. 11. Strategic Plan Review Carolyn Jenkins suggested to have Sanofi look at the data for updates, and questioned if DSC should focus more on schools and plans of care. Also, she mentioned supporting ADA related to children. John Douglas noted that ADA already has a program for kids (in school or after school) for free. Diane Mathews mentioned that we should form telehealth partnerships and check on mini grants. She suggested that DSC contact Jimmy McElligott, MD at MUSC because a lot of equipment has been placed in PCP offices, but it is not being used. Carolyn Jenkins agreed to write a grant that can be adapted to different areas (i.e. pregnancy & pre-eclampsia, etc). A Diabetes Telehealth Task
5 Pillar Evaluation of Meeting 1-5 (1 = lowest and 5=highest) Parking Lot Force was formed. Members include Pam Arnold, Sarah Smith, Carolyn Jenkins, Kathie Hermayer, Diane Mathews, and John Douglas. 6 Diabetes Initiative of South Carolina Outreach Council September 2017 December 2017 Functions As defined by Section amendment to 1976 Code of Laws for South Carolina, the Diabetes Outreach Council shall oversee and direct efforts in patient education and primary care including: 1. Promoting adherence to national standards of education and care. 2. Ongoing assessment of patient care, costs, and reimbursement issues for persons with diabetes in South Carolina. 3. Preparing an annual report and budget proposal for submission to the Diabetes Initiative of South Carolina Board.
