2 NAME & DATE OF INCORPORATION C.A.R.E. (Centre d Activités Récreatives et Éducatives) is our exact name as it appears on our Letters Patents, issued by the government of Quebec in The C.A.R.E. Centre is our short name and named used in English correspondence. GOVERNMENT OF CANADA REGISTERED CHARITABLE STATUS We have registered charitable status with the Canada Customs and Revenue Agency. Our BN/registration number is: RR MISSION Our mission is to enhance the lives and functioning of adults with severe physical disabilities. We are distinctive from other organizations because of the profound degree of the physical limitations of our adult clients. MANDATE Centre d activités récréatives et éducatives (Centre C.A.R.E.) is a non-profit organization whose mandate is to defend and promote the rights and interests of adults with severe physical disabilities living in the Greater Montreal region, including Laval, the Laurentiens and the South Shore through the provision of educational programming and recreational activities adapted to the needs of these people. C.A.R.E. staff and volunteers will work in partnership and collaborate with other agencies and organizations to provide a suitable environment and space to carry out these activities, as well as ensuring that adequate qualified staffing who are trained in support techniques and educational methodologies are in place to protect and serve the special needs of these people. 2
3 WEBSITES Website: Tumblr: Facebook: WHO WE ARE The Centre d'activites Récréatives et Éducatives is a day-centre for severely physically-disabled adults in the Montréal area. We offer a program of recreational and educational activities to adults over 21 years of age with severe physical disabilities. Our programs are offered in English, making us one of the very few Centre s of our kind in Québec. We pride ourselves on the outstanding level of support we offer to our clients. Our aim is to promote autonomy, meaningful social interactions and enhance their quality of life. We operate in a space provided by the English Montreal School Board. Since February 2013 we have been located at the Marymount Adult Centre, at 5785 Parkhaven Avenue, in Cote- Saint-Luc. Our adult day program at the centre is open from 9:00AM until 4:00PM, Mondays through Fridays Currently, we serve 23 clients between the ages of 21 and 60. Our clients come from a range of living situations. Some live with their families, some in group homes, and others in a hospital setting. They are Anglophone, allophone, and francophone. Our staff includes one full time Program Director, a roster of dedicated staff members who fill 5 full-time caregiver positions, and a host of regular volunteers. We have been in existence as a non-profit organization since
4 WHAT WE DO Presently we offer a wide range of recreational and educational activities aimed at continuing education, enhanced communication, social and physical skills. In addition to activities at the centre, we offer outings once a week. Recent outings have included trips to the Musee des beaux arts, the Cote Saint Luc Community Centre, and the DHC Gallery. Due to our high client-to-staff ratio we are able to offer individual help with activities, which allows our clients to participate to the fullest of their abilities. Our activities are adapted to suit our clients needs, and the programs are ever-changing depending on the desires and interests of the participants. RESIPTE FOR FAMILIES In addition to our continuing education program, which operates throughout the academic calendar year (September June) to enrich the lives of our clients, the Centre provides respite for caregiver family members through our respite programs which include our Summer Outings Program a 5-week outings program for our clients to experience summer activities in Montréal. A Summer Respite Program a 3-week, full-time activity at Camp Massawippi for our clients who live on-site with a full team of CARE staff. As well, we also offer a Winter Respite Program a 5 day, full-time winter respite opportunity through the Acces Plein-Air in Chertsey, QC. MORE THAN JUST RECREATION We are able to help our clients contact the necessary resources to help them live full and enriching lives. This help may include adapted equipment, wheelchair repair, and housing information. We work as part of a team with our clients social workers and families to ensure that all of the resources available to them are fully utilized. Our staff is also available to help the clients with individual problems or concerns on a one-to-one basis. On a group level, we are also working with the clients to improve listening, behavioral, and life skills. Our program is one of fun, learning, and growing. 4
5 BOARD OF DIRECTORS We have a Board of Directors currently comprised of 11 members. Our By-Laws allow for up to 12 members comprising client representatives, family representatives, and community representatives. Our classes and activities are adjusted to the abilities, needs, and interests of each student, encouraging maximum independence. We maintain communication with the Mackay Centre School (MAB-MACKAY Rehabilitation Centre) in order to upkeep our waitlist with potential candidates of graduates from the Mackay Centre School who have severe physical disabilities and who may be non verbal and use alternative and augmentative communication methods. In order to best serve our clients and assist them with keeping their personal communication devices, wheelchairs and health needs up to date, we have an ongoing communication with Centre de Réadaptation Constance-Lethbridge and Centre de Réadaptation Lucie-Bruneau as well as Miriam Home and Services and the West Montreal Readaptation Centre. STAFFING Our day program is animated by a team of 4 male staff, and 5 female staff not including the Executive Director. We also rely heavily on the help of our dedicated volunteers, stagieres, interns, and board members. 5
6 WAITING LIST We are pleased to announce that through the continued support of the West Montreal Readaptation Centre (Le Centre intégré universitaire de santé et de services sociaux de l Ouest-de-l Île-de-Montréal) and the English Montreal School Board, we are able to accommodate more and more students as the years go on. Along with the support of the English Montreal School Board, we have 4 classrooms which provide additional space to expand our services. We are aware that there are young adults and families who are unable to find a suitable day program that can serve their unique physical care needs. Every year there are young adults between the ages of 18 and 25 who are inquiring to come to the C.A.R.E. program. Our ideal C.A.R.E. is a place where we can provide services to all those in need and eliminate our waitlist. Our website now has an application form for new clients to apply to be on our waiting list. 6
7 EN COLLABORATION AVEC NOS PARTINAIRES VOLUNTEERS The diverse programs at the C.A.R.E. Centre are in a large part due to the great volunteers that we have. Throughout the year volunteers give more than 2000 hours of their time and this is not including the many hours from the Board of Directors or overtime hours from staff. 7
8 PROGRAMMING In addition to the following programs, clients receive ongoing support with personal issues. The program runs with basic rules of mutual respect and trust. Complete participation within the range of student abilities is mandatory. Client s can create goals focusing on the areas of physical health, social/leisure, psychosocial issues, life skills/social skills, communication, & adult education. PROGRAM ACTIVITIES Daily programming at the C.A.R.E. Centre takes the form of 45 minute group sessions led by the C.A.R.E. staff. The daily program calendar includes 3 program periods, each of which offers 3 choices to clients. The core elements of the programming at the Centre include: Verbal & Written Communication Reading skills, Writing, Poetry Math & Budgeting Health and wellness Media and Visual Arts Physiotherapy and stretching Language arts Community Involvement & outings Feelings & Behaviours Critical Thinking Radio Broadcasting, Public speaking Cooking, Nutrition, Baking, Gardening Current World Events Music, Simple Theory, Instruments & Singing Computer & Technology, Blogging, Science Knowledge & Experiments Swimming, Adapted sports, games, boxing Hands on Art, Srt Theory, Art History Special Guests & Fundraising Personal Goals & Projects Improv, Dama & Expressivness Sewing, Animal Therapy, Pupperty 8

10 ONE-ON-ONE TIME Each client receives 1 hour or more of 1-on-1 project time per week; ½ hour of personal time during which they are allotted a staff to assist them in any activity they choose, and ½ hour of core subject support dedicated to personalized tutoring in the areas of math, reading, writing, and communication. Some personal projects have included communication technology assistance, internet & , arts & crafts, journaling, biography writing & health and wellness education. FUNDRAISING In addition to core funding through the English Montreal School Board and the MSSS, we receive funding from a number of foundations and corporations, and from private citizens through donations, and fundraising events. Centre offers 2 full-time respite initiatives each year: A 5-day Respite Stay at Acces Plein-Air in March, including 24-hour care for 12 clients with food, winter sports programs, specialized outdoor and nature programming and oneon-one time. A 3-week Summer Respite Program at Camp Massawippi in the Eastern Townships in July/August for 13 clients including specialized summer activities, waterfront time, and outdoor camping. As well, the C.A.R.E. Centre offers a 5-week Summer Outings Program that has 2 staff members who bring 4 clients (on a rotating basis, so that every client of the C.A.R.E. Centre can go on 4 outings per summer) to various summer activities throughout Montréal. 10
