Serving over 3,500 families annually, HPP offers case management, prenatal and parenting support, housing assistance, domestic violence services, mental health services, substance abuse support, child welfare support, job training and emergency support of basic needs. The first agency in San Francisco to hire former clients as direct service staff, HPP is unique in that its growth and evolution has been guided, from its earliest days, by the community it serves. HPP s mission is: in partnership with our families, break the cycle of childhood poverty. HPP has received considerable recognition for its service to the community. Most recently, HPP s Founder & Executive Director was named one of twenty-five international 2013 CNN Heroes. The agency has also been highlighted by the New York Times, People and San Francisco Chronicle, and was recently named one of the San Francisco Business Times Best Places to Work in the Bay Area. HPP was identified as one of the highestimpact nonprofits fighting homelessness in the Bay Area by Philanthopedia, a division of Guidestar. E EVENT OVERVIEW HPP s annual Our House celebrates its service to the community by bringing over 400 supporters, community members and government officials together for a jubilant evening filled with live music, cocktails and dancing. Staff-led tours of the agency s vibrant, three-story home offer visitors a behind-the-scenes glimpse at our life-changing work, and former clients return to tell their inspiring stories of overcoming adversity. The event raises over $200,000 to support the agency s mission of breaking the cycle of childhood poverty. This year s Our House benefit will celebrate the agency s 25th Anniversary. To fete this special occasion, the party will be themed to the roaring 20 s. Guests will be encouraged to doll up in their swankiest gowns, kick off their heels and do the Charleston on the dance floor.

5 PROGRAM OVERVIEW HPP promotes healthful living in families with three major goals: 1) Healthy Babies: serve parents as they work to deliver healthy babies and bond with their infants; 2) Safe, Nurturing Environments Where Children Thrive: empower motivated parents with the knowledge and skills necessary to support a child s healthy development and success; and 3) Economically Stable Families: ensure access to information and resources that facilitate the acquisition of permanent, stable housing and support economic self-sufficiency. HPP serves over 3,500 low-income and homeless families through the programs and services: Promoting family stability, HPP s Case Management Services provide on-going support for individuals and families facing various challenges, including domestic violence, substance abuse and parent-child separation. Last year, 3,978 families received case management services. The Wellness Center offers free prenatal education classes, alternative health services, post-partum groups and other activities to over 500 women annually. Additionally, staff provides vital support to expecting mothers overcoming substance abuse through the New Beginnings Program. Since 1992, over 3,500 babies were born to HPP clients. Of those, 90.1% were born at a normal weight and 98.5% were born drug-free. The Housing Assistance Program provides limited, short-term financial assistance and workshops that help families obtain and retain safe and stable housing. Since 2006, HPP assisted 2,187 families with housing. Last year, HPP helped 434 families obtain or retain housing. Of the 207 families contacted at their 9-month anniversary of receiving assistance through the program, 201 (97%) remained stably-housed. HPP s Mental Health Services provide on-site individual, couples, family and group therapy to HPP clients. Last fiscal year, 102 families received therapy from HPP therapists and an additional 45 received on-site parent/child therapy services through the Child Trauma Research Project. HPP s Community Health Worker (CHW) Training Program prepares economically disadvantaged women for meaningful careers in community-based organizations. During this year-long, intensive, on-the-job program, CHWs are trained in office and computer skills, client intake, record keeping and community outreach; attend classes to earn GEDs, advanced degrees and professional certifications; and intern with other agencies for additional experience and exposure to potential employers. Since 1995, 180 women have participated in the program; of those, 150 (83%) graduated and 136 (90%) of the graduates found jobs within 30 days. The Community Technology Center (CTC) offers clients access to and training in computer use, the Internet and other technology to assist clients with job and housing searches. Last fiscal year, 2,153 people visited the CTC. Of these visits, 72% involved employment or housing search (client-reported). The Family Economic Success Program offers free tax preparation services, education in the U.S. Tax Code, assistance with IRS initiated tax issues, and help with obtaining ITINs, as well as support in accessing public benefits. Last tax season, HPP helped clients file 1,208 returns. Total refunded amount was $2,017,523 (up 8% from 2012), with $776,503 in Earned Income Credits and $595,434 in Child Tax Credits. Further, HPP helped 51 clients with ITINs and provided benefits consultation to 128 families. HPP s free, on-site Childcare Center provides quality childcare for children while their parents participate in services. Childcare staff is supported by a Child Development Therapist and a Child Development Consultant. Last year, the Childcare Center saw 761 children through 2,739 visits and assisted 110 families with SF C3 applications. HPP provides Emergency Support of Basic Needs in the form of assistance with food, clothing, diapers baby and children's items and hotel/transportation vouchers in crisis situations. Last year, HPP distributed 184 emergency food bags (in addition to 50 tons of fresh produce in conjunction with the SF Food Bank), 123 grocery vouchers, 65,000 diapers, 190 transportation vouchers, 80 umbrella strollers and 38 car seats to families in need.
