Glands are seen dispersed solid nests are noted in section A with in the connective tissue of (arrow) gland A5/A6 Gleason Score +=6 Distinct glandular units with moderate size lumina. Size variation is less marked. Invasion in to normal glandular Connective tissue is seen in section A5 (Arrow). A7/A8 Gleason Score 4+=7 Predominant pattern is type 4 as glands are angulated, fused and cells in ribbon pattern are also seen. Well formed glands of Pattern are also visible in section A8 (star). Fused glands, including the "hypernephromatoid" pattern, were defined by Gleason as pattern 4. A9/A10 Gleason Score4+4=8 Predominantly irregular cribriform and fused glands (pattern 4, Black arrow) admixed with very few well defined smaller glands of pattern (Yellow arrow). However large majority of sections has pattern 4 involving more than 99% area, score of 4+4 was given to these specimen. Supplemental Figure 1

Gleason pattern type glands seen in section B4 (40X) are separated from each other and relatively smaller in size compare to type glands in section B. However pattern was most predominant among all section to score +. B5/B6 Gleason Score +=6 with extension into adjacent non-neoplastic classification of gleason pattern. Variation in shape and size is a prominent feature. Blood Vessel (arrow). Although the cytoplasmic description is not the part of Gleason grading basophilic cytoplasm is very distinct here (Red Arrow). B7/B8 Gleason Score 5+5=10 Tumor cells are seen in solid sheets and cords without any organized lumina. Poorly differentiated cancer cells with out any glandular architecture is a typical Gleason pattern 5. Most predominant pattern of 5 gives gleason score of 5+5. B9/B10 Gleason Score +=5 Small glands are infiltrating in to the connective tissue of normal gland of Gleason type (arrow) mixed with large sized and closely packed glands of pattern are seen in section B10 gives score of +.

Predominant pattern in both sections is gleason 5 with score of 5+5. C/C4 Gleason Score +=5 Glands with variation in size and shape invading normal prostate tissue favoring classification of Gleason in section C(Arrow). Glands with lesser size variation and glands which are closely packed favors Gleason pattern (star). Predominant pattern in both the sections was however gleason resulting score of + C5/C6 Gleason Score +=6 prominent feature. Blood Vessel (arrow). Although the cytoplasmic description is not the part of Gleason grading basophilic cytoplasm is very distinct here (Red Arrow). C7/C8 Gleason Score +=5 Small glands are infiltrating in to the connective tissue of normal gland. Variation in size is very clear feature (arrow). Although pattern glands are most prominent; well formed large sized and closely packed glands of pattern are seen in section C8 (star) gives score of +. C9/C10 Gleason Score +=6 prominent feature. Cribriform glands of pattern are seen in section C10 (arrow)

Fused, poorly formed and angulated glands of pattern 4(arrow) was second predominant feature favoring score +4
