Fundng levels approved by ths acton would reduce the set asde porton n the FY20 16 Adopted Budget by a lke amount. BACKGROUND: On October 26, 2015 the sub-commttee met and consdered varous Ad to Prvate Organzaton fundng requests that recently came to the Cty outsde of ts newly mplemented annual applcaton process. Detals of the ndvdual requests and the resultng sub-commttee recommendatons are outlned below and n the attached lst of approved fundng requests from that meetng.

2 Requestor: Dunedn Scottsh Arts Foundaton ("DSAF") Amount: $2,500 Intended Use: The DSAF s only hopng to breakeven from ts frst year of operatons. A recent courtesy fre safety nspecton revealed several areas of current noncomplance whch wll cost approxmately $3,320 to rectfy. The requested ad would assst n payng some of these unforeseen costs. Sub-Commttee Recommendaton: Fundng $2,500 Requestor: Famly Center on Deafness (located n Largo, but provdes crtcal servces to Dunedn resdents who are sufferng from hearng loss) Amount: $2,000 Intended Use: Recent changes to the organzaton's Amplfed Telephone Program have resulted n a loss of fundng. The requested ad would help to backfll the fundng defct ths program s facng. Sub-Commttee Recommendaton: Fundng $500 ths frst tme around and re-evaluate subsequent requests f they come n. Requestor: N/ A Amount: $50 Intended Use: Mayor's Councl of Pnellas County Holday Donaton - ths year's donaton wll be pad drectly to Personal Enrchment for mental Health Servces ("PEMHS"). Last year, the Mayor's Councl Holday Donaton of $50 was pad to the Salvaton Army and put through by the Cty Clerk chargng t to the Ad to Prvate Organzaton Account n the Cty Commsson Budget. Sub-Commttee Recommendaton: Fundng $50 Requestor: Dunedn Ppe Band Amount: $10,000 Intended Use: Ths would represent the Cty's contrbuton toward the Band Drummer' s salary. Along wth Pnellas County, the Cty already partcpates n payng for the Ppng Drector's salary by fundng $20,000 n the Other Contractual Servces Account n the Parks & Recreaton Department's Budget for the Communty Center. To be funded usng carryforward of 20I5 unexpended balance of $4,225 n Economc Development ( I80 I) General Fund (00 1) Promotonal Actvtes Acct ( 48-I 0). Durng dscussons wth the Commsson regardng the creaton of an Annual Ad to Prvate Organzaton Applcaton Process, the use of a smple certfcaton that would provde for some accountablty n terms of delverng publc benefts to the Cty was suggested. On October 1, 20 I5 the frst request for payment of FY 2016 grant funds was submtted by the Dunedn Fne Art Center ("OF AC"). In order to move forward wth processng that payment, members of the sub-commttee have developed a smple certfcaton and acknowledgment form whch the Cty could requre organzatons to sgn as a condton of payng out ther grant funds. Wth ths acton, the sub-commttee s nformng the Commsson t also approved both the form of the document and ts use for ths purpose at ts meetng held on I 0/26/20I5. Lastly, n more recent dscussons regardng Ad to Prvate Organzatons, the concept of staff mplementng a smlar process for handlng perodc requests for Cty sponsorshps (small dollar or n-knd) was suggested. These types of requests could be added to the tems revewed by the sub-commttee at meetngs t convenes, and ts recommendatons on sponsorshps then brought back to the Commsson, along wth any recommendatons t s makng n connecton wth Ad to Prvate Organzaton fundng at the tme.

The DSAF sgned a lease agreement wth the Shloh Baptst Church at the end of September 2015, terms statng a 5-year perod at $7 50 per month, to ncrease by $100 per month/year $1,250. We expect wth utltes, nsurance, lawn/pest control, we estmate an addtonal $400 to $500 per month. In order to rase the money needed to sustan the buldng's operatng costs, we have plans to use the buldng as a source of revenue wth the followng: Host ndvdual and group lessons n bagppes, Scottsh drummng a nd dancng. (Already begun: have held approx. 10 classes n October) Host nstructonal camps and workshops for the above Hold events: rectals, concerts, holday partes Our Jr. Commttee (teenagers) hold events/dances (Already booked an event for Chrstmas) Rental of hall to others for Celtc weddngs, famly gatherngs, partes (Already have our frst rental) Rental of hall to others for meetng space Membershp program recently launched wth annual renewals The buldng was nspected pror to sgnng the lease, and a recent courtesy walk-thru nspecton by Deputy Fre Marshal Kelly lntzes. A report was delvered on Frday, October 23rd, and we are already workng to complete areas of non-complance. 6 Ths past weekend we nstalled sx commercal paper towel dspensers that were donated, and replaced/repared several batteres and bulb ssues. One of our members purchased ktchen cabnets at the Habtat for Humanty overflow store n Palm Harbor, and donated them to the Foundaton. Our members are commtted to helpng the Foundaton grow. We are workng hard to save a dme wherever we can, knowng that all our profts go back nto the Cty's Scottsh programs, and are proud to have gven out over $64K n donatons n Year We are prepared not to make any money n our frst year of operaton, but are hopeful we can rase enough funds to break even. As we get the buldng "rent-ready", and rase awareness, we are confdent we wll be a success! Ths grant request comes wth a promse that the Dunedn Scottsh Arts Foundaton wll make the Cty of Dunedn very proud. By offerng the nstructons and programs on Scottsh musc, and expandng nto other forms of Scottsh vsual and performng arts, we are workng to rase awareness of the Cty's hertage. '\.. ~..., (])unedln S cottsf }l rts - -FOUNDATION-- September 23, 2015 Dear Mayor and Cty Commsson, I am delghted to announce that after a lot of hard work, the Dunedn Scottsh Arts Foundaton {formerly the Dunedn Hghland Games & Festval Commttee) has secured a buldng n whch we wll be able to call home! We just sgned a lease for the property located at 1134 Douglas Avenue, whch wll serve as a headquarters for our Foundaton, run our own Ppng, Drummng and Hghland Dance Classes, and become a place to hold fundrasng events and gatherngs. Ths would not have happened wthout the assstance of Cty Manager Rob DSprto and our Lason, Vce Mayor Heather Gracy, to whom we owe our sncere thanks. In order to get ths new venture off the ground we would lke to respectfully ask the Cty for help wth the expense of settng up the buldng. We are hereby submttng ths request for a grant award n the amount of $2,500, whch we wll use towards the setup costs n our frst year. Ths new faclty wll offer us the opportunty to add to the number of best-n-class muscans and dancers n Dunedn, who wll go on to be the next generaton of Cty Ambassadors as they take ther sklls to compete around the country. The Scottsh Arts are a sold part of the fabrc of Dunedn, and our ntenton s to buld on that wth an expanded program for the students n the areas of art, lterature and poetry, as well as the more tradtonal Scottsh Arts, and hope that the Cty wll help us to get ths off the ground. We are plannng to host a formal Open House to open our doors very soon, and be sure to share the news wth you. Once agan, thanks to Rob and Heather, and hope that the entre Cty Commsson wll support us n ths request. FCD serves the varous needs of resdents wth all degrees of hearng loss. We are commtted to mprovng the qualty of lfe for the nearly 140,000 Pnellas County ndvduals wth hearng loss, whch ncludes resdents n the Cty of Dunedn. We have been provdng our servces n Pnellas County snce In 2015, FCD provded the followng servces to Dunedn resdents: FREE new amplfed telephones, tranng, telephone modfcaton, and/or exchange of equpment After school programmng for elementary, mddle, and hgh school students, and summer camp Support groups for parents and famly actvtes Sgn language, deaf culture and parentng classes Tutorng and homework help Educaton on hearng loss and preventon Socal and mental health counselng Case management Whle we do receve fundng from other agences, t does not meet all the needs of provdng servces to Pnellas County resdents. Many of the chldren and adults we serve are lvng well below the poverty level. Wthout our organzaton's servces, they would have lmted access to the specfc sentces people wth hearng loss need. Recent changes to our amplfed telephone program have resulted n a loss of fundng. If we cannot fnd addtonal mones to make up the defct n ths program, we wll be forced to alter or cut amplfed telephone servces. We ask that your muncpalty consder makng a contrbuton n the amount of$2, to our agency to help us defray our addtonal costs.
