1 Personalized Training Request Form Date Name Member # Birthdate Desired Dates & Times Phone Desired Instructor (staff bios on back) Please note the following: 1. Personalized Training is sold in package's of three, one hour sessions. If you would like half hour sessions, please discuss that with your instructor and you will be able to have six sessions. 2. We will NOT be issuing "partial refunds" for forgotten sessions. It is your responsibility to show up to each scheduled session and if you cannot make it you must call and cancel. 3. All sessions must be cancelled 24 hours prior to the time the session is scheduled. If you do not cancel 24 hours prior to the session, then that session counts as one of your sessions. If prior cancellation hasn't been made and you are more than 15 minutes late, then that counts as one of your sessions. 4. Online scheduling reminders are now available. Please provide your address so we can get your account set up. 5. Any scheduled sessions not paid for will be cancelled will be canceled after 6 months. 3 ROSEMARY LEIBERMAN Rosemary is a certified through the National Association of Sports Medicine. Her passion and focus is on older adults seeking to improve their health and well being. As we age, there is a natural decline in our muscle tone, balance and flexibility. Rosemary s focus is on keeping our physical abilities and improving them for a continued healthy lifestyle. Rosemary is a Yoga, Pilates and Silver Sneakers Instructor. She has been teaching hatha yoga for close to 15 years and has experienced participants health increases through exercise. 4 CHERI HAYS Cheri has been working in the fitness industry since 2010, and as a swim instructor since 1996 and has many years of experiences coaching. You may also see her lifeguarding, teaching, or training in our small group area at the YMCA. She really enjoys sharing her enthusiasm for fitness with others. 5 KIM DALMATOFF Kim started her fitness journey many years ago but has recently became Certified as a Personal Trainer. Once she obtained her certification, she realized how many other opportunities there are in fitness. Kim is also a Certified Spin Instructor and Body Pump instructor. She loves having fun while helping others be active and get in shape to reach their goals. She has worked to help condition an athlete who just wanting to improve their form, increase their stamina, then a incorporated a HIIT/ Crossfit style which is her favorite. She also has the ability to scale workouts to basic foundational movements to help those recovering from injury to feel strong and confident again in a safe manor. 6 SARAH JACQUES Sarah is passionate about helping her clients, no matter the age reach all their goals and their best self. She believes fitness is accessible to everyone no matter who they are. Fitness is life changing and guides people to live healthier, and happier lives. This is not one size fits all fitness, it is individualized, functional and life changing for the everyday athlete and warrior that is in us all. When Sarah is not here at the Y she is, hiking, kayaking, paddle boarding or out enjoying a plethora of other fun out door activities she loves, as well as cooking up a storm and working on her mental fitness and physical fitness. 7 RYAN SNIDER Born and raised in Ventura, Ryan comes to us with a long history with working with athletes and clients of all ages. He started coaching high school basketball in 2008 and started coaching CrossFit and overall Strength and Conditioning in 2010, while holding a USAW certification and CrossFit Level 1. He has worked with high school athletes (water polo, swimming, track and basketball) and with general health and wellness athletes to help achieve their goals. Ryan will push your limits to reach any fitness, health, or lifestyle goal you set out to reach. No matter your goals, putting on lean muscle mass, shedding those last few inches, or just feeling better Ryan will coach you to your best yet!
8 CHIA SANTANA Francisco Chia Santana is a world class professional boxer who has been seen on HBO, ESPN, Showtime and Fox Sports. The former WBA International Champion was the first boxer to win a title from Santa Barbara, California and he is currently one of the top rated welterweights in the entire country. Chia recently relocated to Ventura with his family and even though he s fought at legendary venues like Madison Square Garden in New York and The MGM Grand Garden Arena in Las Vegas he s now here at the Ventura Family YMCA. * Certification Here s your chance to be trained by a famous professional athlete in their physical prime!
9 TAYLOR BARNES Taylor began her health and fitness journey shortly after graduating college with her degree in Culinary Arts. With a love for dancing as her foundation, she quickly became a Zumba Instructor. Helping others stay fit quickly fuled her passion for movement towards helping others. She became a certified personal trainier through NASM.
