A two-day conference session is planned for Thursday, April 24, 2014 and Friday, April 25, 2014, with an award reception on the evening of Thursday, April 24, All registered attendees of the Annual Meeting will receive complimentary access to the reception. The Annual Meeting will offer scientific sessions in a variety of formats including large plenary sessions and smaller concurrent sessions. Sessions will focus on issues and topics relevant to Building Health Equity through Collaboration. The Conference is a one of a kind event in NYS where public health professionals from all settings can participate in learning opportunities and skill-building programs; develop a rich networking community; engage in public health activities designed to enhance personal growth and renewal as well as benefit the communities we all serve. Continuing Education We are pleased to offer continuing education credits at the 2014 Annual Meeting at no additional charge. Continuing Education credits for Certified Health Education Specialists (CHES) are under review for entry or advanced-level credit hours for the two-day event. Continuing Education credits for Certified in Public Health (CPH) are under review. Professionals seeking continuing education credits not listed above will be provided a completion certificate upon request for self-submission.

7 Hotel Information All events will take place at the Gideon Putnam Resort in Saratoga Springs, New York. A block of rooms has been reserved for Wednesday, April 23, 2014 and Thursday, April 24, Please call (518) to reserve a room by March 24 th under the group code 9N48KH for a group rate of $104/night for standard rooms or $154/night for suites. Transfer/Cancellation Policy If you registered for the conference and cannot attend, you may transfer your registration to another person. All transfer/cancellation requests must be submitted in writing and received by NYSPHA no later than April 18, Please submit your request via to info@nyspha.org. Cancellations received on or before April 18, 2014 will be refunded less a $10 administrative fee. Cancellations received April 19, 2014 or later are not refundable. Space at each event is limited no shows will be charged the full registration fee. Important Information Nearest Airport- Albany International Airport is the nearest airport to the event. Participants arriving via air may rent a car on-site at the airport or take a taxi. The Gideon Putnam offers shuttle service from the airport to the resort at a rate of $60 each way. Please call the hotel at (518) for more information. The airport is approximately 20 miles from the conference site. Train Station- The nearest train station is the Saratoga Amtrak station. The Gideon Putnam offers complimentary shuttle service to both the Saratoga Amtrak train station and downtown Saratoga. Hotel Parking- Parking is free at the Gideon Putnam Resort. Food & Beverage- Lunch on day 2, coffee breaks and an evening reception on day 1 are included with your registration fee. The Gideon Putnam s chef will design a menu to meet the Healthy Meeting Guidelines recommended by the New York State Department of Health. Gluten-free options will also be available. For those registered for the pre-conference workshop on April 24 th, lunch will be provided. Conference Attire- Please keep in mind that meeting room temperatures vary.
