Describe the impact of palliative care on patient health; 2. List the top four health outcomes that older adults value; and. Outline the biggest demographic shift in the U.S. Palliative Care: Pharmacist Opportunities and Perspective : Ebetesam Ahmed, PharmD 1. Describe the roles and responsibilities of the pharmacist in palliative care; 2. Discuss the barriers to medication discharge process;. Assess, recommend, and treat pain and common symptoms encountered in the palliative care setting; and 4. Identify problems with medication use in the home and how pharmacists can help. Lunch Break Nutrition and Palliative Care : Edwina Rich, MS, RD, CDN 1. Describe the role of nutrition care and nutrition support in overall patient care; 2. List nutrition interventions in palliative care;. Outline the role of palliative care in patient symptom management; 4. Describe the team approach to patient care; and 5. Explain the importance of nutritional screening and assessment for palliative care patients. Rehabilitation Therapy and the Palliative Care Population : Miriam Gross, PT, DPT, NCS 1. Distinguish between the contributions of physical therapy, occupational therapy, and speech language pathology with the palliative care population; 2. Identify appropriate consults for rehabilitation therapies;. Differentiate between traditional rehabilitation therapy and rehabilitation therapy for the palliative care population; and 4. Describe the role of rehabilitation therapy on the interdisciplinary palliative care team. The Essential Coherence of Palliative Care and Social Work : Terry Altilio, MSW, ACSW, LCSW 1. Outline the Institute of Medicine and World Health Organization palliative care reports as a framework for administrators and practitioners to lead efforts integrating primary palliative care across settings and diagnoses; 2. List the domains of palliative care and link them to social work roles. Demonstrate the essential congruence of palliative care with the principles and values of quality social work practice; 4. Identify principles and skills which link primary health social work and other professionals involved in the provision of palliative care; 5.
