3 SUMMARY General objectives Urban Health Centres 2.0 (UHC2.0) promote innovative integrated health and social care pathways, early detection of frailty, management of polypharmacy and prevention of falls for active and healthy ageing in European cities. The general objective is to develop, implement, and evaluate the UHC2.0 in 5 varied European cities that will act as pilot site, in order to produce and disseminate a transferable and easily implementable model for the innovative UHC2.0. The UHC2.0 should be a platform for the realisation of the priorities of the European Innovation Partnership on Active and Healthy Ageing. In the whole project, a strong communication and optimal dissemination strategy will be realised. Strategic relevance and contribution to the public health programme We aim for a reach of > 70% of older people invited for preventive care; implementation integrity of >70%; a half standard deviation (SD) improvement in measures of independence and HR-QoL; a decrease of use-of-care and costs after 14 months of follow-up compared to the baseline measurements; and finally, an appreciation by stakeholders >7 on a 1-10 scale. The project will result in a transferable and easily implementable model for UHC2.0 for integrated health and social care pathways, early detection of frailty, management of polypharmacy and prevention of falls for active and healthy ageing in European cities. A tool box with instruments and protocols and a concise policy document will be produced that contributes to the priorities of the European Innovation Partnership on Active and Healthy Ageing (EIP-AHA). Methods and means UHC2.0 is set up to design and implement actions that support the pillar on prevention, screening and early diagnosis of the Strategic Implementation Plan adopted by the Steering Group of the European Innovation Partnership on Active and Healthy Ageing (EIP-AHA). UHC2.0 focuses on innovations with regard to the primary care setting at the local level. UHC2.0 contributes to the following actions of EIP-AHA: Prescription and adherence action at regional level, Personal health management, starting with a falls prevention initiative, and Action for prevention of functional decline and frailty. The objectives of UHC2.0 are in direct alignment with the objectives of the 2nd Health Programme of Community Action in the Field of Health ( ), in particular the promotion of health, including the reduction of health inequalities and the generation and dissemination of health information and Page 3/11

4 knowledge. Expected outcomes period Knowledge regarding universal and context specific characteristics of UHC2.0, as well as demands/preferences of older people and other stakeholders will be used to produce a generic European UHC2.0 template. Existing evidence-based interventions regarding detection of frailty, integrated care pathways, management of polypharmacy, and prevention of falls, supported by ICT tools will be identified, optimised and integrated in UHC2.0, while the generic UHC2.0 template will be adapted to local preferences/context in each pilot city. The adapted/completed UHC2.0 will be implemented in Rijeka, Pallini, Valencia, Manchester, Rotterdam, and applied to (net) citizens at each pilot site. Nurses/assistants do screening of frailty, and a shared decision will be made regarding integrated care pathways that will be applied and monitored, including management of polypharmacy and fall prevention, with 14 months follow-up. The CDC-Framework for Programme Evaluation will be used applying 2 perspectives: end-users (older citizens); and care providers/services/volunteers. The design: a specific pre-post controlled design. Measurements at baseline, and during/after 14 months follow-up. Integrating the results, a tool box with instruments and protocols and a concise policy document will be produced to disseminate the innovative UHC2.0, to realise the priorities of the EIP-Active and Healthy Ageing.
