Th application will b trminatd or tabld for failur to appar at a schduld public haring, without first providing writtn notic to th Public Srvics Dpartmnt. If you dsignat an agnt or attorny to rprsnt you in this application, you must submit a compltd Agnt Affidavit in a form approvd by th City.
