5 REGISTRATION FOR ANNUAL SESSION ASDA Members: $699 prior to March 15, 2018/$799 after March 15, 2018 Nonmembers: $799 prior to March 15, 2018/$899 after March 15, 2018 Dental Anesthesiology Residents*: $295 (no increase in fee after March 15, 2018) *Must be enrolled in a dental anesthesiology residency program and be a current Resident member of the American Society of Dentist Anesthesiologists Other Dental/Medical Residents: $395 (no increase in fee after March 15, 2018) Pre-Doctoral Students: $100 (no increase in fee after March 15, 2018) REGISTRATION FOR CRICOTHYROTOMY SESSION** SATURDAY, MAY 5, 2018 Meeting Attendee Fee: $ (Residents: $150.00) **Limited to 40 participants! First come, first served. Once the session is full, we cannot accept additional participants. NOTE: This session will run concurrent to the General Session lectures. There is no additional CE offered for the Cricothyrotomy Session.
