3 Family Membership & Donation Drive It is that time of year again! As many of you know, our business year runs from October 1st through September 30th. It takes approximately $3,000 to $4,000 of income each year to maintain our Family Association budget. Our Family Association funds numerous significant projects including the Employee Recognition Program and Educational Opportunities for our Family Association Members. Every year we ask the members for an annual fee of $15.00 per household. We are encouraging our members to pay their fees as soon as possible! This year we have seen a large decrease in dues being paid. In order to continue the Family Association s projects, a total of $2,500 needs to be collected. This number is an absolute minimum to keep the group functioning. We are also requesting any donations if at all possible. We would like to encourage all family and extended family/friends of our New Hope clients to become members! For a small annual fee you will receive our Family Association Newsletter, all important information regarding events, and can stay active and informed about your loved one. On behalf of New Hope, a sincere Thank You to the current Family Association Members for all of your support and dedication! Refer to Page #4 to view current paid members. Please use the form at the bottom of this page to ensure proper credit for your membership. Please include your address at the bottom of the form as we would like to utilize as an additional contact source. Dues are $15.00 per household for the membership year Oct. 1, 2015 Sept. 30, 2016 NAME: ADDRESS: CITY, STATE, ZIP: NAME OF FAMILY MEMBER WHO RECEIVES SERVICES FROM NEW HOPE: RELATIONSHIP (PLEASE CIRCLE): PARENT SISTER BROTHER OTHER MEMBERSHIP AMOUNT PAID: $ DONATION AMOUNT PAID: $ FAMILY ASSOCIATION NEW HOPE FOUNDATION P.O.
