April 04, 2018 New Funding Opportunity: No Wrong Door System Business Case Development ACL is releasing a new funding opportunity to help states measure and quantify the impact of their No Wrong Door (NWD) systems. Funds will specifically be used to identify and evaluate data that demonstrates how person-centered counseling and evidence-informed interventions, such as care transitions, nursing home diversion programs, and the Veteran-Directed Home and Community Based Services (VD- HCBS) program, empower individuals to access quality long-term services and supports in ways that meet their needs and delay or prevent the use of more costly services. Additional project goals include working collaboratively with ACL and its partners to develop a methodology and toolkit for calculating a return on investment (ROI) for NWD systems. Grantees will be expected to share key data elements with ACL to test and calculate ROI at a national level and produce a business case model that demonstrates the impact of NWD systems. Grants up to $600,000 each budget year will be awarded to up to nine states for a 24 month project period. 7 ACL: Webinar: Addressing Social Determinants of Health in Primary Care Cecily Slasor, Administrative Assistant April 10, 2018 Webinar: Addressing Social Determinants of Health in Primary Care Register for April 24 webinar. Although a growing body of research suggests that social determinants of health social, functional, environmental, cultural and psychological factors are intricately linked to health and wellness, our fragmented medical and social services are often underequipped to address these needs. The Ambulatory Integration of the Medical and Social (AIMS) model developed by the Center for Health and Social Care Integration (CHaSCI) at Rush University Medical Center integrates masters-prepared social workers into primary care teams to identify, address, and monitor social needs that influence health. Preliminary evidence indicates that AIMS reduces clients emergency department visits, hospitalizations, and readmission rates. AIMS also creates opportunities for community-based organizations (CBO) to develop partnerships with local health clinics to integrate care and promote better health outcomes. Please join the Aging and Disability Business Institute on April 24 at 1:00 PM Easternfor a one-hour webinar. This webinar will highlight training and implementation support for CBOs interested in replicating AIMS. Participants in this webinar will be able to: Articulate areas needing health and social care integration; Identify key elements of the AIMS intervention; and, Identify the opportunities and challenges of building partnerships between CBOs and health organizations. 9 April 4, 2018 Webinar: Safe and Effective Use of Medications in Older Adults Register for April 18 webinar. Beneficiaries dually eligible for Medicare and Medicaid have on average, a 25 percent higher rate of chronic conditions than beneficiaries who are not dually eligible. Dually eligible beneficiaries also tend to use a variety of medications and have higher Medicare Part D prescription drug costs than Medicareonly beneficiaries. Addressing medication management, medication misuse, and adverse drug effects among dually eligible beneficiaries is important for the health of these individuals. Please join Resources for Integrated Care for a webinar on April 18 at 12:00 PMEastern. The speakers will provide an overview of these important medication issues related to older adults, along with the need for management and coordination among the care team members and the beneficiary. Participants will also learn about effective strategies to empower individuals and their families to manage multiple medications for maintaining their health. Participants in the webinar will be able to: Define polypharmacy and the implications for patient safety; Name at least five classes of medications that are known to have a high risk of adverse reactions in older adults; and Recognize challenges that beneficiaries and their family members may experience when managing multiple medications, and define how each member of the care team can assist with medication regimens. 11 On behalf of CMS s Center for Medicaid and CHIP Services, Mathematica Policy Research is seeking public comment on the measure specification and justification for a quality measure currently under development. The measures are called: Improving or Maintaining Physical Health in Younger Dual Eligible Adults and Improving or Maintaining Mental Health in Younger Dual Eligible Adults. A memo listing questions for public comment, as well as the measure information forms (MIF) and measure justification forms (MJF), are available in zip files on the webpage under "List of Currently Accepting Comments". The public comment period for these measures is open until May 10, Please submit your comments to MedicaidQualMeasures@mathematica-mpr.com.
