1 July 30, 2006 To: All MEADA Supporters Dear MEADA Supporter, The Methamphetamine Education and Drug Awareness (MEADA) Coalition of Wright County, Minnesota is hosting Minnesota s first medical conference specifically directed toward medical professionals. This conference, named Medical Minds & Meth, will take place from 7:45 am to 5:00 pm on October 18, 2006 at the Minneapolis Convention Center in Minneapolis, Minnesota. We are looking for commercial support and monetary grants to mitigate the costs involved in hosting this conference. This is an opportunity to participate in the first national conference on Meth Education that is specifically geared towards top medical professionals. As an event supporter, you will have access to interact with dynamic medical professionals from around the nation, you will receive acknowledgement in our program and have the opportunity to join us at a vendor fair open during breaks and meals. This conference features two nationally known speakers as well as an array of breakout sessions covering topics from prevention, to treatment options and intervention. This high level event will bring a new level of knowledge and awareness to the medical field. In addition, registrants will earn Continuing Medical Education Credits by attending the full day. 2 FREQUENTLY ASKED QUESTIONS Who Is MEADA? Methamphetamine Education and Drug Awareness (MEADA) Coalition of Wright County, MN, was developed to educate youth, families, and citizens on the dangers of drugs with a focus on methamphetamine. This coalition was started in January Its members include the County Sheriff, the County Attorney s Office, the County Human Services office, the County Public Health office, the County Court Services office, and members of the local governments, cities and townships. The goal of MEADA Coalition of Wright County is to bring together families, neighbors, school personnel, law enforcement, county agencies, faith communities and municipalities to help develop a county mindset of zero tolerance for chemical use with a special focus on methamphetamine. You can learn more about MEADA online at What is the purpose of the Medical Minds & Meth Conference? This conference is aimed at educating medical professionals on the signs, symptoms, prevention, intervention and treatment options available to Meth addicts. In addition, we hope to increase awareness on the neurological effects of this addiction and the costs to the community. Who is your target audience? This is the first conference in Minnesota to discuss Meth addition at a high level and specifically target the medical field. Our attendees will be Physicians, Physician Assistants, Nurse Practitioners, Registered Nurses, and Mental Health Professionals. How many will attend? We are mailing invitations to at least 20,000 medical professionals, including associations, clinics, and hospitals. We expect at least 500 medical professionals from across the Midwest will attend.

This is a one-day conference scheduled to take place at the Minneapolis Convention Center from 7:45 am to 5:00 pm on October 18, It includes three general sessions and three breakout sessions. Continental breakfast and lunch is included in the cost of attendance. A detailed schedule is attached. What about Sponsorship Opportunities? Because we are offering Continuing Medical Education Credits to the attendees of this event, we are only asking for commercial support and monetary grants for this event. This conference is for educational purposes and will not promote any specific business interest of any commercial interest. Any commercial support or grant monies will be dispersed at the discretion of the event hosts (MEADA). Commercial promotion, including product and promotional materials and advertisements are prohibited in or during the CME activity. Outside of the education space, promotional materials may be displayed or distributed. To support our commercial supporters, we are holding a vendor fair, which will be open during registration, breaks and meals (see agenda attached).

4 COMMERCIAL SUPPORT OPPORTUNITIES PLATINUM LEVEL - $10,000 Limit of Four Includes two 8-foot tables at the vendor fair, your company logo on the conference bag, your marketing materials placed in the bag given to every attendee near check-in. You will also be acknowledged in other printed pieces produced for the seminar, including the brochure mailed out to at least 20,000 potential attendees and receive a mention at lunch (July 15, 2006 deadline for brochure) GOLD LEVEL - $5,000 Includes one 8-foot table at the vendor fair as above, your marketing materials placed in the bag given to every attendee near check-in, and your company logo on printed pieces produced for the seminar. SILVER LEVEL - $2,500 Includes one 8-foot table at the vendor fair as above and your marketing materials will be placed in the bag given to every attendee near check-in. BRONZE LEVEL - $750 Includes one 8-foot table at the vendor fair. 7 Medical Minds & Meth Conference Flag Avenue N Phn: Fax: COMMERCIAL SUPPORT AGREEMENT MEDICAL MINDS & METH CONFERENCE OCTOBER 18, 2006 The Meth Education and Drug Awareness Coalition of Wright County, MN (MEADA) is committed to presenting CME/CEU activities that promote improvements or quality in healthcare and are independent of the control of commercial interests. As part of this commitment, MEADA has outlined in this written agreement the terms, conditions, and purposes of commercial support for its CME/CEU activities. Commercial Support is defined as financial or in-kind contributions given by a commercial interest i, which is used to pay all or part of the costs of a CME/CEU activity. This activity is for scientific and educational purposes only and will not promote any specific proprietary business interest of the Commercial Interest. 2. The Accredited Provider is responsible for all decisions regarding the identification of educational needs, determination of educational objectives, selection and presentation of content, selection of all persons and organizations that will be in a position to control the content of the CME, selection of education methods, and the evaluation of the activity. Appropriate Use of Commercial Support 3. The Accredited Provider will make all decisions regarding the disposition and disbursement of the funds from the Commercial Interest. 4. The Commercial Interest will not require the Accredited Provider to accept advice or services concerning teachers, authors, or participants or other education matters, including content, as conditions of receiving this grant. 5. All commercial support associated with this activity will be given with the full knowledge and approval of the Accredited Provider. No other payments shall be given to the director of the activity, planning committee members, teachers or authors, joint sponsor, or any others involved with the supported activity. 6. The Accredited Provider will upon request, furnish the Commercial Interest documentation detailing the receipt and expenditure of the commercial support. Commercial Promotion 7. Product-promotion material or product-specific advertisement of any type is prohibited in or during the CME activity. The juxtaposition of editorial and advertising material on the same products or subjects is not allowed. Live or enduring promotional activities must be kept separate from the CME activity. Promotional materials cannot be displayed or distributed in the education space immediately before, during or after a CME activity. Commercial Interests may not engage in sales or promotional activities while in the space or place of the CME activity. 8. The Commercial Interest may not be the agent providing the CME activity to the learners. Disclosure 9. The Accredited Provider will ensure that the source of support from the Commercial Interest, either direct or in-kind, is disclosed to the participants, in program brochures, syllabi, and other program materials, and at the time of the activity. This disclosure will not include the use of a trade name or a product-group message. The acknowledgment of commercial support may state the name, mission, and clinical involvement of the company or institution and may include corporate logos and slogans, if they are not product promotional in nature. The Commercial Supporter and MEADA agree to abide by all requirements of the Accreditation Council for Continuing Medical Education (ACCME) Standards for Commercial Support of Continuing Medical Education (appended).
