W also want to improv accss to GP appointmnts so vryon who nds an urgnt appointmnt can do so quickly. This includs urgnt car for both mntal and physical halth. In Sundrland, this currntly includs: NHS th numbr to dial for nonlif thratning mdical concrns. It is a national tlphon numbr, whr halthcar profssionals ar availabl to provid tlphonbasd clinical advic, accss to out-of-hours doctors and widr mdical srvics whn thy ar ndd. Thr will b an improvd srvic in plac from Octobr Mor halthcar profssionals will now b availabl to giv advic to patints. This mans that mor popl can call to gt halth advic without th nd to travl to an appointmnt. This will b known as th Intgratd Urgnt Car Srvic. Pharmacis thy offr advic for minor illnsss and injuris as wll as advic on th saf us of mdicins. Lots of pharmacis ar opn lat and at wknds. Thr ar 63 pharmacis in th Sundrland ara, so it is nvr far to travl to on. Pharmacists ar xprts in th us of mdicin and abl to diagnos and trat many common illnsss. GP srvics during normal working hours - Th GP is whr most popl go whn thy hav a halth problm. In Sundrland thr ar 40 GP practics, mostly opn Monday to Friday, 8am to 6.00pm. GP srvics during vnings and wknds (xtndd accss) sinc 1 Sptmbr 2017, GP appointmnts hav bn availabl wkday vnings from 6.00pm to 8.30pm, and on wknds and bank holidays. GP practics hav com togthr to offr ths appointmnts and th srvic is availabl in up to fiv locations to all patints in th city. Appointmnts ar mad via your own practic and NHS 111. GP srvics out of normal working hours - Th GP out-of-hours srvic aims to provid accssibl urgnt car whn GP practics ar closd. Dpnding on th svrity of th halthcar issu, patints may b offrd tlphon advic, an appointmnt or a hom visit. Urgnt Car Cntrs - At Sundrland s four urgnt car cntrs (somtims known as walk-in cntrs), GPs and nurss s patints who hav a halth issu which is not lif thratning but nds to b sn that day. Thy ar opn Monday to Friday 10am to 10pm and from 8am to 10pm, Saturday, Sunday and bank holidays. 5

Popl want joind up car and thy would prfr to s thir GP for sam day urgnt appointmnts. Dmand kps on growing Ovr th last tn yars, dmand for halth srvics has continud to grow. Historically additional srvics wr opnd on by on to cop with incrasing dmand and this is why th urgnt car cntrs wr opnd. Urgnt car cntrs wr dsignd to tak th strain away from accidnt and mrgncy (A&E) but this is not working. Mor and mor popl ar still going to A&E (now calld th mrgncy dpartmnt). Two in thr popl who go to A&E, ithr don t nd tratmnt or ar transfrrd back to thir GP (using data from January to Dcmbr 2017). W nd to do somthing diffrnt, as w cannot kp srvics as thy ar now. Th A&E mrgncy dpartmnt nds to b kpt fr so popl with lif thratning mrgncis can b sn quickly. This is on of th rasons why th nam A&E has bn changd to mrgncy dpartmnt. W nd to spnd our mony mor wisly W hav too many srvics that ar providing similar srvics at th sam tim. W nd to mak sur that w spnd mony wisly across th NHS systm in ordr to scur NHS srvics for th futur. Srvics ar also availabl at Sundrland Royal Hospital This causs confusion about th bst plac to go and is not a good us of our limitd NHS rsourcs. 6

Also th Univrsity of Sundrland is to opn a school of Mdicin in 2019 National policy As a CCG, w hav to follow national guidanc and policis, st by NHS England. NHS England has said that urgnt and mrgncy car must b rviwd to nsur: Car is suitabl for popl of all ags with physical and mntal halth issus Each ara must hav an urgnt tratmnt cntr (mor information on pag 17). W also mad sur w took into account what patints wr tlling us. W want to put a systm in plac whr patints ar sn whn thy nd mdical hlp, at th bst location for thir nds and by a halthcar profssional with th spcialist skills to trat thir condition. Any improvmnts that ar suggstd nd to b managd within th rsourcs that ar availabl to us. 2 Hlp popl to incras slf-car (looking aftr yourslf) through accss to appropriat mdical advic Many minor illnsss or injuris gt bttr without th nd for profssional mdical hlp. For halthy popl most common illnss, such as coughs, cold and flu, will clar up in a fw days with a combination of rst and mdication from a wll-stockd mdicin cabint. By giving popl information about thir own halthcar nds, this will hlp popl dvlop th knowldg, skills and confidnc to manag minor halthcar issus thmslvs. With th right information, many popl can also monitor and control long-trm conditions such as diabts or asthma. This will allow mor popl to liv thir normal day-to-day livs without rquiring mrgncy appointmnts. This in turn hlps fr up rsourcs so that mdical profssionals ar availabl for thos who rally nd mdical hlp. 10
