4 The 14 th Annual Meeting Intended Audience Physicians, surgeons, researchers, genetic counselors and registry coordinators involved in caring for patients with inherited colorectal cancer syndromes. Progam Goals 1. Improve quality of care for patients with inherited colorectal cancer through enhanced interdisciplinary collaboration and communication. 2. Increase identification and screening of patients at risk for hereditary colorectal cancer. 3. Create treatment plans for hereditary colorectal cancer patients using the latest clinical research. 4. Educate the healthcare community regarding familial risk factors that suggest hereditary colorectal cancer and encourage the utilization of registry expertise in diagnosis, management and research. Abstracts Submissions for abstracts must be received by July 17, More abstract information can be found at How to Register Attendees are given the opportunity to register online at or by using the attached registration form. Please follow the instructions on the website carefully. Upon receipt of the registration fee for the conference, each attendee will receive a registration confirmation letter. The registration fee includes a digital version of all program materials, breakfast and lunch on Tuesday and Wednesday, and a poster reception and one ticket to the Lifetime Achievement Award Dinner, Tuesday, October 12 th. Early registration ends September 10, After that time, registration will be accepted based on availability. Cancellation Policy Refunds minus 30% for administrative costs, unless specified differently in the conference description, will be given for cancellations received in writing a minimum of seven (7) days before the conference. Nationwide Children s Hospital reserves the right to cancel any program listed based upon enrollment or other contributing factors. Individuals who have enrolled will be notified at least one week in advance of a cancellation. Full tuition will be refunded or can be applied to another program should a program be cancelled. Continuing Education Medical: This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education through the joint sponsorship of Nationwide Children s Hospital and the Collaborative Group of the Americas on Inherited Colorectal Cancer (CGA-ICC). The Nationwide Children s Hospital designates this educational activity for a maximum of 13 AMA PRA Category 1 Credit(s). Each physician should claim credit commensurate with the extent of their participation in the activity. Genetic Counselor: An application is being submitted for Genetic Counselor Continuing Education Units (CEUs) through the National Society of Genetic Counselors (NSGC). Please check the registration website for the most up to date conference CEU information. Disclosure of Commercial Relationships Anyone in a position to control content must disclose all relevant financial relationships with any commercial interests in the last 12 months. Any relationships identified will be resolved prior to the individual s involvement in the educational activity. Full disclosure will be made to participants.
