11 LOCAL CARDIAC SUPPORT SERVICES GWENT HEALTHCARE NHS TRUST Name Description Contact Details Heart Call Voluntary Home Visiting support Heart Line Telephone Information service Heart Call is a voluntary home visiting support service for heart patients and their families living in the Caerphilly and Pontypool area. The volunteers have all experienced heart problems themselves and are recruited and managed by Gwent Healthcare Trust and are supervised by the Cardiac Rehabilitation team. Initial training and subsequent monthly training is provided by the Trust. Visits, if required, are arranged on discharge from hospital by the cardiac rehabilitation nurse. The volunteers receive travelling expenses. Heart Line is a telephone help line manned by trained volunteers who have experienced heart problems themselves. Volunteers are recruited and managed by the Gwent Healthcare Trust and are supervised by the Cardiac Rehabilitation nurses. Initial training and subsequent monthly training is provided by the Trust.
