1 PHONE: (972) LaDonna Courtney November 16, September 30, 2005 LaDonna Courtney, formerly LaDonna Fields, 51, of Frisco, TX, passed away on September 30th, having lived a life of achievement and contribution cut short by cancer. She is survived by her devoted husband, Charles Harrison, father and stepmother Eugene & Betty Courtney, sister Rhonda (Wayne Scott), stepchildren Grant and Paige Harrison, 7 nieces and nephews, 10 great-nieces and nephews, 3 step-sisters, an adoring extended Texas family, and a vast array of loving friends and admirers. She was preceded in passing by her mother, Estelle Courtney, and brother, Terry (Pat) Courtney. For the 11 years prior to her move to Texas a year ago, LaDonna was Executive Vice President of the AZ Academy of Family Physicians, which honored her extraordinary service by establishing the LaDonna Courtney Certificate of Commendation, awarded to a non-physician who makes an outstanding contribution to forward the specialty of family practice. In the early 90s, LaDonna was Executive Director of Valley Citizens League. Before that, she was an executive assistant to the Maricopa County Board of Supervisors. Before and after her graduation from Valley Leadership Class XIII in 1992, she was an active participant in numerous boards and organizations, including the Coalition for Tobacco Free Arizona, the Arizona Tourism Alliance, Center for New Directions, and Business & Professional Women, serving as president of several. A Certified Association Executive, LaDonna was a Board member of the AZ Society of Association Executives, where she chaired several committees and held all offices on the Board before becoming President. She was named 2001 Executive Director of the Year. This year she will be the third person ever to receive the AzSAE Angel Award, recognizing an individual who has demonstrated dedication and outstanding support of AzSAE and the association management profession. LaDonna brought grace, competence, joy and humor to her contributions to her profession, our Valley, and all who knew her. A life celebration will be held at 4:30 p.m., Sunday, October 2, 2005, with visitation from 7-9 p.m. on Saturday, October 1, both at Turrentine- Jackson-Morrow Funeral Home, 2525 Central Expressway North at Ridgeview Drive (Exit 38 on I-75), Allen. Arizona services will be held at Unity of Phoenix, 1500 E Greenway Pkwy in Phoenix at 10:00 a.m., Wednesday, October 5, Viewing will be held at Hansen Mortuary, 8314 N 7th St, Phoenix from 6-8 p.m. on Tuesday, October 4th. In lieu of flowers, the family would appreciate donations to: AZ Academy of Family Physicians Foundation, 5320 N 16th St., Phoenix, AZ

2 Memorials I went to Cahokia high school in Cahokia, IL with LaDonna. She was an awesome person, friend, and classmate. She will be missed on this earth. She is in a better place now with our Lord and Savior Jesus Christ. LINDA J. RODENBERG, SEPTEMBER 6, 2010 LaDonna and I were close friends in high school. I can still hear her sweet laugh today even though I haven't seen her in years. She taught me how to play a tune on the piano when were 17 "The Enchanted Sea". I can still play it today. BTW, that is only tune I can play on the piano with two hands. I played it for her at our 30th class reunion. Thank God I did, that's a wonderful memory that we shared that night. I am so sad I only found out about her passing 5 years after the fact. My love and condolences to her family. As LaDonna's fellow chapter executive from Ohio, I enjoyed her friendship as well as benefited from her professional expertise and advice. She was a wonderful person and I feel privileged to have had the opportunity to know her and as friend and colleague. My sincere and heartful sympathies to her family and friends. The courage, love and devotion to her life in efforts to conquer it by her husband and love of her life, and among family members and friends was astounding, but not surprising. Whether you knew her a long time, a short time, very well or casually, healthy or ill, LaDonna had a spirit that touches you and stays with you. 6 school, LaDonna was one of the first individuals I contacted. I had been involved with the Colorado Academy of Family Physicians while living in Denver, and wanted to maintain my relationship with AAFP through the Arizona chapter of AAFP upon arrival. LaDonna was very warm in welcoming me to AZ, and I wound up becoming the AAFP membership liaison for my school (AZCOM). LaDonna was always very supportive to me as a medical student, and when I started applying to residency programs (in family Medicine, of course), she wrote me a wonderful, touching, and beautiful letter of recommendation that I will never forget and that I will cherish forever. It was the strongest endorsement for me as a future Family Physician that anyone -- including my professors at school and my preceptors -- could have written. I was moved to tears by it. But this exemplifies, in my opinion, what LaDonna was all about. She was a very very good person who was always willing to go out of her way to help others. Now that I am at the Good Samaritan Family Medicine Residency program in Phoenix, AZ and on my way to becoming a Board Certified Family Physician (although still an intern), the influence of LaDonna becomes all the more remarkable. I will never forget her, nor the words she wrote about me in that wonderful letter of recommendation. It will be my goal in life to try always to live up to the view that LaDonna had of me as a future Family Physician...to be the best I possibly can be. I know that LaDonna will be sorely missed by everyone who knew her, by everyone who crossed paths with her during her lifetime. I send my sincerest condolences to her family and to her loved ones. You are in my thoughts and prayers at this difficult time. I will miss her and think of her often. My time with her, and her words to me, will always be a part of me.
