2 Mission & Areas Served Statements Mission Statement At Susan G. Komen, our mission is to save lives and end breast cancer forever by empowering others, ensuring quality care for all and energizing science to find the cures. Background Statement Susan G. Komen fought breast cancer with her heart, body and soul. Throughout her diagnosis, treatments, and endless days in the hospital, she spent her time thinking of ways to make life better for other women battling breast cancer instead of worrying about her own situation. Moved by Susan's compassion for others and commitment to making a difference, Nancy G. Brinker promised her sister that she would do everything in her power to end breast cancer. Though Susan lost her battle with the disease, her legacy lives on through the work of Susan G. Komen, the organization Nancy started in her honor. Komen is the global leader of the breast cancer movement, having invested more than $1 billion since its inception in Komen s promise is to save lives and end breast cancer forever by empowering people, ensuring quality care for all and energizing science to find the cures. Across the country, that promise is upheld by a network of over 100 local Affiliate offices. At the heart of each Affiliate is a person or group of people who, like Susan, wanted to make a difference. Affiliate History Susan G. Komen Greater Kansas City began with a Race for the Cure event in 1994 organized by three inexperienced, headstrong women who relocated to Kansas City from Washington, DC with a can do attitude. In those days, a small core of committed women kept files in their homes and cars, had all Race phone calls on an answering machine in a spare bedroom and treated Komen as their second job. It was truly a grassroots effort. The need for management grew increasingly evident over the years. Thus, the first Board was formed and an office was generously donated in Today, Susan G. Komen Greater Kansas City serves 10 counties in Kansas and 7 counties in Missouri and has a staff of seven while continuing to have nearly 200 active volunteers serving on various committees. With the help of passionate volunteers, a dedicated board and staff members, Komen Kansas City has given back over $19 million in support of our mission since $11 million has been granted to local organizations working in breast health and $3.7 million has been given in support of global research efforts. $4.4 million has been invested in local educational events, outreach programs, referrals to services and public policy efforts. 2

3 Impact Statement Susan G. Komen is the boldest community fueling the best science and making the biggest impact in the fight against breast cancer. We impact breast health throughout our 17-county service area, including northeast Kansas and northwest Missouri through research, community health, and advocacy all of which are made possible through our fundraising efforts. Susan G. Komen Greater Kansas City is proud of the work which has been done in the local service area since our inception. In 2015, Komen Kansas City published our most recent Community Profile. The Community Profile is a full needs assessment which provides direction to the affiliate for targeted initiatives, funding efforts, and more. This profile takes significant resources to complete but is used by many interested in the breast health of the service area. We are also proud of our newest signature event, "Rock the Ribbon" which brought nearly 1,600 supports together for our common cause. We were also excited to launch our Connected for the Cure program which brings breast health education to the community in a new, more personal format. In 2016, we look forward to many additional successes. Komen Kansas City will begin the work outlined in our community profile, such as creating a campaign to educate the community on accessing breast health services, whether that be through assistance programs or the provisions of the Affordable Care Act. Komen is also taking on new endeavors to connect with survivors in ways that are meaningful along their journey, based on suggestions of current survivors. Komen will also expand our Connected for the Cure program to be a resource to business and individuals in a way that they can easily learn the basics of breast health with the ability to connect with a breast health expert who can navigate them to any resource they may need, including free or reduced costs mammograms and personal risk assessments. Needs Statement Susan G. Komen Greater Kansas City Affiliate is always open to partnering with individuals and businesses who are passionate about further the mission of the organization. Currently, we are seeking the following: Qualified Board Members Grant Reviewers Funding for a large-scale education campaign around access to breast health services Funding to assistance with bringing a breast cancer issues conference to the area Partners to utilize our Connected for a Cure program in their place of business, worship, or home CEO/Executive Director/Board Chair Statement Susan G. Komen Greater Kansas City is part of a movement that has successfully increased the five-year breast cancer survival rate from 74% to 99%, and decreased deaths by 33%. Locally, our first Race for the Cure was held in 1994 and since then, we have granted over $19 million back to our mission. Komen is extremely proud to fight breast cancer on so many fronts with an innovative approach based on community feedback and data. Unique to our organization is our Community Profile which is an extensive needs assessment of our 17 county service area. The results of this assessment serves as a guide for strategic planning at the affiliate and is at the core of everything we do. Particular to the most recent profile, we learned that our community wants to be inspired to take action although many of them wait until they are desperate-when a lump appears, pain in their breast becomes too strong to ignore, etc. We are taking on the challenge to find new ways to connect with our community to inspire them to get screened for breast cancer while continuing to find ways to make access to breast care easier and more affordable. Additionally, we strive to be a continual resource long after diagnosis, to help survivors of breast cancer transition to new life stages. 5 Programs Programs Race for the Cure Description Category The Komen Kansas City Race for the Cure is an annual 5K Fun Run/Walk that attracts nearly 20,000 people. Our participants both run/walk to save lives, but additionally participate in an outstanding Race Expo experience to include food-beverage-product sampling, company information and services, live entertainment and more. Race Day is about awareness, empowerment, and most importantly, celebrating life. Diseases, Disorders & Medical Disciplines, General/Other Cancer Population Served Adults,, Short-Term Success Long- Term Success Program Success Monitored By Examples of Program Success Funds raised during the Race for the Cure directly impact the amount of grants awarded each year. Grantees provide screening, education, and treatment programs which has directly impacted those who's cancer has been detected early. The Komen Kansas City Race for the Cure is our foundation's signature event. The Race is held to increase awareness of breast cancer in the community and to bring together advocates in a joint effort to raise funds for community programs and ultimately find a cure for breast cancer. Evaluations are sent out to sponsors, committee members and volunteers after the Race in an effort to determine changes for the next year. A lot has happened in the last two decades. After the first Race for the Cure in Corporate Woods in July of 1994, Komen Kansas City presented its first grants in February $42,000 was given to Truman Medical Center and $42,000 was given to the The University of Kansas Hospital. In past years, grant funds have reached the level of just under $1million. That means even more help for medically vulnerable women, men and their families in our 17 county service area. During these challenging times, that has a real impact. This remarkable growth would not be possible without the commitment of over 20,000 participants each year. 5

6 Grant Program Description While we have made great advances over the last several decades in understanding the causes of breast cancer, how it behaves in the body, and successful treatments, there are many people who still do not have access to regular mammograms or quality breast cancer treatment. Our grant programs are designed to make systemic, lasting change in our community. The Komen Kansas City grant program enables nonprofit organizations to implement innovative, culturally appropriate, and comprehensive breast health programs in the communities where they live, work, play, and worship. We are proud to have invested more than $11 million into our service area to date. Category Medical Research, General/Other Cancer Research Population Served People/Families of People with Cancer,, Short-Term Success Long- Term Success Program Success Monitored By Examples of Program Success Seventy-five percent of all money raised in the Greater Kansas City area stays local and is used towards our mission, including grants to 501(c)(3) organizations that provide breast health education, screening, and or treatment programs. The remaining twenty-five percent of the funds raised in Kansas City goes to our national organization for breast cancer Research Grants. The Greater Kansas City Affiliate of Susan G. Komen for the Cure has the responsibility to raise funds to support grants that educate, screen and treat women in the Kansas City area on breast health. Grantees are required to submit two (2) progress reports throughout the grant year. Grantees also receive an onsite visit for a Komen Representative and must demonstrate funds are being used for the purposes granted. Every few years, a community needs assessment survey is performed in our 17-county service area. The outcomes can be compared to past grant programs to see if improvements have been made in the areas of need that were identified two or three years prior. Grantees are held accountable to wisely use the dollars given. Komen's Community Health Director talk with the Grantees regularly to make sure they will be successful. 6

7 Community Outreach & Mission Education Description Susan G. Komen Greater Kansas City believes in empowering the community to make breast health choices based on the most up to date health information and recommendations. Our signature breast selfawareness message is shared with the public through a variety of formats. Connected for the Cure: an interactive experience that uniquely educates participants on breast health, connects them to screening resources such as no-cost mammograms and risk assessments, and compels them to take action after experiencing the story of a breast cancer survivor through virtual reality. Breast Care Hotline: members of the community are encouraged to call our hotline to receive assistance with their breast health needs and questions. Examples of typical needs include: free or reduced costs mammograms, assistance with wigs, prescriptions, etc., or questions regarding screening recommendations. Pink Army: Komen's Pink Army is made up of volunteers who attend local community events and provide breast health information, either through one-on-one interactions or speaker format. Komen volunteers and staff are involved with each year. Also, another benchmark for the program is the expanding number of volunteers who are actively engaged. The goal in creating many avenues of outreach is to develop a grass roots alliance of well-informed advocates, working in our communities to engage in education and information sharing through public speaking and public forums. The program provides the mechanism for full community engagement. Susan G. Komen for the Cure relies on thousands of volunteers to deliver the message of breast health and carry out the promise of the organization. The Greater Kansas City Affiliate reaches out to thousands of people in the metro, teaching the importance of regular breast exams and annual mammograms. Throughout the year, a growing number of Komen volunteers are speaking to groups about breast health. Ultimately, the long-term goal for this program is to educate the Kansas City Community about the importance of breast health. This includes being aware of risk for breast cancer, annual clinical exams, and annual mammograms. Additionally, the affiliate wants the community to understand the signs and symptoms of breast cancer as well as make healthy lifestyle choices. The Susan G. Komen for the Cure promise is to save lives and end breast cancer forever by empowering people, ensuring quality of care for all and energizing science to find the cures. We believe this promise will me made by EDUCATING the general public on the importance of breast health. 7

8 Program Success Monitored By Examples of Program Success The affiliate monitors success of our community outreach through tracking mechanisms including outputs as well as successful intervention evaluations. Evaluations include pre/post tests with Connected for the Cure and pledge cards for better breast health. The breast health hotline is monitored by the amount and type of calls assisted. Breast Care Hotline Success: In September of 2015, Laura reached out to our breast care hotline stating she was "just diagnosed and feeling very overwhelmed." Our Community Health Director, a social worker, began a conversation with Laura that lasted for several months. Laura did not have any family in town other than her husband who worked a large amount of hours during the week and a son with special needs who she was the primary caregiver of. Laura was given resources where she could receive wigs, prescription assistance, case management and more. Laura utilized the resource funded by Komen and reported that she left the agency feeling supported and was excited to share that during her wig fitting, she able to let go and just have some time to enjoy being a woman again. Laura was also invited to attend Race for the Cure through our survivor code which provided a free registration for her. Most of all, Laura was able to just share her thoughts with someone throughout the day as needed. During packet pick up for the race, Laura was able to meet our Community Health Director in person and share how the support received was able to help her when she needed it most. CEO Comments As we continue to analyze community feedback, we have found the need to re-energize the community and educate them in new ways. Attending health fairs is no longer the best way to reach the public. We have worked hard to raise funds and create new touchpoints for the community through programming. We are also working diligently to learn more about our community of breast cancer survivors and how we can work to meet their unique needs at all points of life after diagnosis. 8

Each year, the Affiliate helps thousands of women and men with little or no health insurance receive breast health education, screening, and if diagnosed, to quality, effective treatment and treatment support services.since our inception we have invested more than $19 million into our mission.up to 75% of net proceeds generated by the Affiliate stay in the Greater Kansas City region.the remaining 25% is dedicated to the Komen Award and Research Grant Program to fund groundbreaking breast cancer research. Komen Kansas City conducts a needs assessment to guide our priorities and work more effectively within the community.the needs assessment includes an overview of the demographic and breast cancer statics that highlight target areas, groups or issues. Komen uses that need assessment to make informed decision on how the Affiliate can use its resources to make the greatest impact on breast cancer in the area through the awarding of local grants, by increasing funds raised through Komen 14
