2 Mission & Areas Served Statements Mission The Cancer Support Community of Southern Connecticut (CSC-SCT) is an affiliate of the Cancer Support Community an international network of independently chartered nonprofits that provide professional programs of emotional support, education and hope for people affected by cancer, at no charge to participants, so that no one faces cancer alone. The Cancer Support Community was formed by the unification of The Wellness Community and Gilda s Club, and the network now consists of more than 50 local affiliates, 100 satellite locations and online programs. CSC-SCT is proud to be building the first facility and off-site capacity of its kind in Connecticut, serving Fairfield, New Haven, Middlesex and New London Counties. Background CSC-SCT offerings are all evidence-based and professionally led. Our program has a framework designed by a national team of experts, and will be implemented locally by a licensed health and wellness professional who will adapt our portfolio of offerings to members needs. We maintain a gold standard of psychosocial cancer support, recognized around the world as the hallmark of the Cancer Support Community. This commitment allows medical professionals to refer their patients without hesitation, building a strong alliance between our efforts and theirs, and ensures that all members of the community have access to the highest quality and full spectrum of services. The CSC-SCT target population is expansive and inclusive. Our programs are designed to serve anyone affected by cancer - men, women, children, teens, patients, survivors, caregivers, friends and loved ones. We accept members affected by any type of cancer. We accept members at any stage of their journey, from the moment of diagnosis, to years post treatment or families living with loss. Our members are the experts in identifying their support needs; our professionals help them navigate the appropriate options. All CSC-SCT programs are offered free of charge to members. Our program has a framework designed by a national team of experts, and will be implemented locally by a licensed health and wellness professional who will adapt our portfolio of offerings to members needs. The Cancer Support Community program has five core components: Support- Licensed professionals work with each member individually to develop a personalized plan. We offer in-person group sessions, topical networking groups for different types of cancer, and online support options. Education- Includes lectures and workshops about cancer and its treatments as well as programs that enhance life skills and coping. All are presented by experts. Healthy Lifestyle- Includes mind/body programs, exercise classes, and nutrition workshops. Social Opportunities- Includes varied activities from jokefests to knitting circles to book clubs to potlucks and evolves with the preferences of the active community. Information and Referrals- Includes a resource library and computer access for self-directed research, as well as referrals to local services. 2
3 Impact Cancer has staggeringly high incidence rates, nationally and locally. Nationally, looking at all forms of cancer, over the course of their life, men have a 1 in 2 chance of developing an invasive cancer and women have a 1 in 3 chance of doing so. The American Cancer Society estimates that 1.5 million cases of cancer are diagnosed annually, with about 20,000 new cancer diagnoses made in Connecticut each year. In 2009, Yale University School of Public Health conducted a needs assessment for cancer-related services in Southern Connecticut, funded and supported in part by CSC-SCT. Healthcare professionals, community leaders, cancer patients and survivors were interviewed to assess their knowledge and opinions on the resources available in Southern Connecticut. Research participants were asked to identify gaps in services currently available in the local area. As a result of this assessment, the need for a Cancer Support Community facility in Southern Connecticut was confirmed. Specific areas of need included: guidance to patients in navigating psychosocial support services, programming for caregivers, cancer survivor support programs, programs that cater to the diverse populations of cancer patients and survivors, access to and integration of alternative therapies into cancer treatments, information and guidance on nutritional options for patients, body image counseling for both men and women, and knowledge and access at all stages of cancer, including post-treatment. Needs The board of directors has set a target date of mid-2014 to launch full program activity and operational capacity in Connecticut. This includes the opening of an anchor facility in eastern Fairfield County or western New Haven County we are currently evaluating locations for ease of access, functional capacity and costs. Given the demographic and geographic realities of southern Connecticut, the board, with approval from the international CSC network leadership, will collaborate with partner organizations to bring program content and services directly into Connecticut communities extending from Danbury to New London. As we are requesting funding for the launch year, based on the implementation process established by the more than 50 open affiliates, the first year will have certain one-time objectives such as site selection for the base of operations. More generally, each year we will measure success based on the number of people who benefit from our program (CSC members or participants) and partnerships with existing organizations and facilities throughout the state -- hospitals, community centers and other local partners. Specific needs include board members, volunteers, and other people who would like to help. From our launch year (2014) budget, are the following specific needs: Rent and Maintenance $60,000 Insurance $5,000 Office Supplies and Postage $11,000 Telephone and Utilities $10,000 Office Equipment $18,000 Database and Website $5,000 Design, Printing, Marketing $18,000 Program Materials $25,000 Audit and Legal Fees $7,500 Training and Conferences $11,000 Other Fees $1,000 CEO Statement Backed by evidence that the best cancer care includes emotional and social support, the Cancer Support Community offers these services to all people affected by cancer. Likely the largest professionally-led network of cancer support worldwide, the organization delivers a comprehensive menu of personalized and essential services. Because no cancer care plan is complete without emotional and social support, the Cancer Support Community has a vibrant network of community-based centers and online services run by trained and licensed professionals. This distinguished track record of helping people living with cancer is much needed here in Connecticut. 3
