2 Mission & Impact Statements Mission To improve the health of rural Tennesseans through advocacy, communication, education, and legislation. Background The nonprofit formed when the state's leaders recognized that there was no organization in TN that dealt with all issues concerning rural health. The vision is to be the trusted resource and effect a positive change in the health and well-being of all rural Tennesseans. We do that by: 1)Promote equitable access to appropriate and comprehensive health services for rural Tennesseans. 2)Maintain a diverse membership that represents all Tennesseans. 3)Empower members to assist the community in identifying and resolving their own unique health care needs through grassroots efforts. CEO Statement We are the only state organization that focuses solely and exclusively on rural issues in Tennessee. We are facing a healthcare crisis across the nation and in Tennessee. We have had 8 hospitals close in Tennessee and this means our rural citizens are faces with fewer access points to receive quality healthcare with longer distances to travel. Rural lives are places at risk when communities lack access to hospitals for emergency care. 2

Budget 4500 Category Health Care, General/Other Population Served,, CEO Comments An Executive Director is in place to create alliances with partners who can increase awareness of continued access to rural healthcare. Education of the challenges rural communities face needs to be shared with state leaders. We now have opportunities for educational webinars and community gatherings to talk with communities and help carry the message of their unique rural healthcare needs to leaders who can help maintain access. One challenge RHAT faces is for additional funding that will allow the Executive Director to plan for these educational opportunities. In addition, we are serving an aging population that face sever chronic conditions. The overall health rankings of our rural communities lag behind the urban rankings in every category. This is exacerbated by the fact that Tennessee is already at the bottom of the category in overall health rankings. When you slice the data and just look at those counties with a rural designation, the picture is even worse. The Rural Health Association of TN is committed to assisting with improving our overall access to healthcare and the overall population health in our rural communities. We must focus on driving the message and resources down to our local leaders to start shifting the cultural norms in our rural counties. 3

This will be a 3-year plan. We have identified areas where we wish to place an increase emphasis and will be working in the following areas: Connect: To create and provide ongoing opportunities for stakeholders to engage in healthcare dialogue, education and advocacy Advocate: To be a vocal advocate for rural health and an expert in rural health issues among policy makers with the ability to impact rural healthcare Educate: To build together the individual passions of our members and partners to create a stronger, more unified voice able to impact healthcare decision making and to speak up for the rights of rural Tennesseans RHAT will host a Rural Healthcare Leadership Symposium where stakeholders will identify barriers and discuss legislation that could effect rural healthcare access and equality of care. We will work to prioritize the issues and move forward with a unified voice for policy changes or legislation to improve rural healthcare. 5

6 Management Executive Director/CEO Executive Director Term Start Aug Mrs. Rebecca Jolley MBA Experience Rebecca is a Partner with AES, LLC and serves as Vice President of Healthcare Consulting Services. She brings with her 14 years of health care and hospital association management experience. She worked diligently from for the Louisiana Hospital Association to address community needs. During her tenure at the association she also developed the Center for Rural Health Excellence through engaging rural health stakeholders statewide to leverage resources and strengthen Louisiana s rural health safety net providers. She negotiated contracts that established a statewide rural health group purchasing network to provide lower cost alternatives to rural standalone facilities and supported lobbying efforts on issues affecting health information technology and rural hospitals at the state and federal legislature. Ms. (Bradley) Jolley also successfully advocated for the enactment of legislation to provide financing mechanism to stabilize hospital share of state s $8 billion Medicaid budget through Constitutional Amendments. She assisted in directing improvements to the engagement of grassroots advocacy groups to broaden the effectiveness of healthcare advocacy and hopes to do the same for Tennessee in her new position. Britt Title Executive Assistant Experience/Biography CEO Comments The Rural Health Association of TN is excited to have Rebecca (Bradley) Jolley as it's new Executive Director and look forward to the coming years with her in that leadership role. Rebecca is an experienced rural health non-profit executive, she has been compelled to pursue her passion of working for and with the rural healthcare safety net to protect and expand access to necessary healthcare related services in rural Tennessee. She brings an extensive background and passion for rural healthcare. After serving the rural healthcare delivery system of Louisiana for over a decade, her prior experience and skills will enable her to take this association to the next level in member services. In addition to her experience in Louisiana, she has spent the past year and a half working with healthcare associations nationwide in a consulting capacity to build successful grant and forprofit programs for state healthcare associations. Rebecca has a proven track record in her ability to engage key stakeholders at all levels to drive to meaningful results that bring value to the rural healthcare delivery network. Ms. Bradley has also been engaged at the 501(c)(6) level of association management as a registered lobbyist, in the 501(c)(3) capacity through building out grant networks and at the for-profit arm through developing strategic business relationships that add revenue to the bottom line of associations. In order to retain this position, it is necessary for RHAT to identify additional funding streams.the recent hiring of a new Executive Director with proven grant writing experience will directly impact the ability to obtain new revenue. Additionally, the annual conference continues to be a large part of the financial support for the association. Our board is preparing a plan for additional sponsors who have the same mission as ours, to improve the health of Tennessee. A new design in our web page will allow us to reach an additional group of potential members who are like mission minded. Because of our long-time success 9

10 for annual conferences, we have the potential to offer this service to others as a revenue source. The increase in revenue will help to solidify the financial opportunity to continue with the Executive Director position at a fulltime level and allow her to build partnerships with others who have a passion for rural health.
