2 BACKGROUND & NEEDS Impact Statement Helping rescue dogs in Manatee County with providing food and everyday essentials. To donate and help rescue organization and shelters with skin issues, burns, sores, allergies with our Aloe Vera Cream called "Ruff Spots. Issue an annual fund (Jack's Fund) to families who have rescued a dog pay for unforeseen circumstances to give their dog a second chance at life. Needs Statement 1. Assist dogs with Veterinarian needs and grooming needs to get them healthy and happy for their new family 2. Provide training and emotional support during their transition 3. Provide food and other needed supplies to get them comfortable 4 Provide transportation if needed Background Statement Ruff Life Charities was born out of an experience where a community came together to save a rescue dog named Jack in It became my mission to help other families and to "Pay it Forward". We have sent up a fund in Jack's name after his passing in 2015 which will be issued on an annual basis. Our main mission has shifted in FL with the need to help more in our local community by assisting the elderly and Hospice patients get the care need for their dog and assist them through the grief process. Statement from the Board Chair Statement from the CEO/Executive Director I personally was faced with a difficult financial decision in 2009 when I rescued my dog Jack and I didn't have the money needed to give him the $5,500 surgery to save his life. The community in Hilton Head, SC heard my story and raised the money needed to save Jack. I will never forget hearing other stories in the Animal Hospital where families were maxing out credit cards, refinancing cars and taking second mortgages out on their homes to get the chance to save their dogs, their family. Never had I experienced people understanding my story, relating to me and Jack and wanting to help unconditionally. I vowed to "Pay it Forward" (just like the movie) and so I started Ruff Life Charities to help others who can't afford expensive surgeries, Veterinarian care and food when they have faced hard times. Through the generosity of strangers is how my life changed forever, help me keep the spirit of our unconditional love going. 2

4 PROGRAMS "Ruff Spots" helping dogs with skin conditions Description As part of my journey, I have been involved with the healing wonders of Aloe Vera and how it helps humans and pets. We have developed an Aloe Vera skin creme safe for dogs and it provides healing to many skin conditions we find all over the US. We are donating our creme to many local rescue groups and organizations to help rescue dogs/cats with skin irritations, sores, allergies, burns and hot spots. We introduced "Ruff Spots" to our market this past month and have donated our creme to many rescue groups in the Sarasota/Manatee County areas. We are receiving incredible results and plan to donate more as soon as we can get our product made. We are a very active Board of Directors who makes the decisions on the cases we help and organize the necessary paperwork and guidelines needed to chose our cases. We are provided with the proof of adoption, phone numbers with their Veterinarians, and other pertinent files needed to determine what help we are willing to fund. We are getting before and after pictures of the results of our Aloe Vera creme posted on our website. We are accepting donation on behalf of local dogs who need additional care in regards to major skin conditions. So far we have donated over $500 of creme to local shelters and will expand to many areas in FL and the US 4

5 Providing Care for Hospice Patients Dogs Description Providing temporary care for rescue dogs for the elderly, disabled or adults faced with an accidental medical situation is become a growing concern. When faced with a broken bone or emergency hospitalization to a person who doesn't have help for their dog, we would provide them with dog walking and/or overnight care while they are not able to provide this for their pet. If this turn into a long term situation, we can help find family member who can care for the dog or find another home if needed. This is a situation that keeps growing and needs to be addressed before the patient is left with no alternatives. Whether it be transporting the animals to family members, Vet care for the dogs, emotional and physical support for the pet through the transition and placing the dog into a new family. We are going to find the solution need for that patient and dog. Long Term Success We have just been approved as a 501C3 non profit charity in July 2013, this is a new program. Our long term goals are to help as many families here in FL in the next year and if our program succeeds will expand our help to other counties in FL. 6 Jack's Fund Description Our charity started from my rescue dog Jack needing emergency surgery in Our community raised the money needed for his surgery and finding a way to "Pay it Forward" became our mission. With Jack's passing in July 2015, we have set up fund in his name to help more families who come into a accident or unforeseen circumstance with their rescue dog to give him/her a second chance with life. Each year we will grant a family the money spent on emergency care for their dog. We want to continue this program in Jack's name after his passing in We have raised over 10,000 to help families in need and will continue this program each year to help more. It was the purpose of starting our charity and will continue in years to come. We contact the Vet offices directly and develop a relationship with the families in need for our screening. Program Success Examples We raised over $1000 for Sophie in Gulfport, FL who was a rescue dog and her mom had been laid off her nursing job due to cancer. Sophie has developed some tumors and other health issues that we helped her with. Lavender was a rescue dog who was in the system for over a year. She was foster by a family familiar with her breed and nursed her back to health and adopted her. She did not get assistance for her Vet bills through the shelter because of funding so we donated over $1000 to her family because of the love and care she got. 6
