2 Mission & Impact Statements Mission To discover treatments and a cure for ALS, and to serve, advocate for, and empower people affected by ALS to live their lives to the fullest. Background Our Chapter first came about in 1987 with the Central Tennessee Chapter, along with the Mid-South Chapter (Memphis) and officially became a single corporation when the Central Tennessee Chapter, Mid-South Chapter and the Tri-Cities Support Group merged in 2005 becoming as we know today The ALS Association Tennessee Chapter. Our staff includes the CEO, Vice President, Finance & Administration, Development & Special Events Manager, Development & Communications Manager, Development Manager, Care Services Director and Care Service Coordinators, who all work together to raise much needed funds and provide services and support for our Chapter families. The staff is guided by a Board of Trustees. Devoted volunteers and community partners who provide invaluable support in all areas of the Chapter. Impact In the quest to create a world without ALS, our vision is to care for and support all people living with Amyotrophic Lateral Sclerosis in Tennessee as we leave no stone unturned in our relentless search for a cure.we will achieve our vision by upholding our commitment to be the:preeminent organization and catalyst in funding and promoting ALS research.most comprehensive provider of care and support services to the Tennessee community through our Chapter and Clinics.Leading advocate and voice for the Tennessee community.recognized authority and most trusted source of information and education about ALS. Greater awareness around the disease and impact being made in the community. 4. Community collaboration to continue to support ALS patients, their families and caregivers across the state of Tennessee. Other ways to donate, support, or volunteer ALS isn't unique to Tennessee, but the level of compassion most certainly is. That's why we offer so many different ways to contribute. The first thing you should know is that the money we raise in our Chapter stays right here to help Tennessee patients and their families. (The only exception being the dollars that are directed toward national and global ALS research efforts, which you can choose to designate.) Receiving no government assistance for our programs, and no operational endowments, our Chapter is funded entirely through donations and fundraising events. Which means we rely on the generous donations of people like you to enable us to provide our services free of charge to ALS patients and their families. The financial cost to families of persons living with ALS is exceedingly high. In the advanced stages of ALS, the cost of care can average $200,000 a year. That's why your donation can make such a difference and why we offer so many different ways for you to become involved with the ALS Association. However you choose to help, we thank you wholeheartedly for your support and interest in enhancing the quality of life for your Tennessee neighbors and families living with ALS. The challenges seem endless, and the resources are many times not enough. It is our hope that through your support, some of these resource gaps will close and we will be able to offer another lifeline of support to everyone touched by this disease. 3

4 Programs Programs Home Visits Description Completed by Patient Service Coordinators for ALS Patients and their families. Each patient receives a visit per year to help access current needs and locate resources. Budget Population Served Adults,, Home Modification/Equipment Loan Program Description Limited financial assistance is available for home modification projects (bathroom renovation, ramp building, etc.) or equipment. The chapter also has a loan closet with equipment that can be used by patients free of charge for as long as needed. Budget Population Served, Adults, Respite Care Description Provides relief from the duties of care giving to the primary caregiver of the ALS patient. Budget Population Served Adults,, Communication program Description The Chapter has working relationships with Speech & Language Pathologist who assist with ongoing adjustments to communication devices. They also make recommendations for assistive devices to enhance communication. We also maintain a limited collection of devises in our Loan Closet Budget 9000 Population Served Adults,, Support and Support Groups Description We know how important it is to support PALS and their families as they live their lives with ALS.
