There are many ways to provide financial support to the MS Society: support an event participant, become a member of the Golden Circle giving club, make a planned gift, donate through workplace giving, donate appreciated stock, enlist your company to sponsor an MS Society event or provide a matching gift for your donation, and create a memorial or tribute fund for someone special. Volunteer opportunities at the Mid America Chapter range from one-time events and service projects to year-long commitments. Internships with the Mid America Chapter provide students with a solid educational framework, as well as opportunities to gain professional experience and make a difference in the lives of people affected by multiple sclerosis. As a Bike MS or Walk MS participant, you will have the opportunity to ride or walk in honor of a person living with MS.

3 Mission & Areas Served Statements Mission Statement The National MS Society's vision is a world free of MS. The Society's mission statement is: We mobilize people and resources to drive research for a cure and to address the challenges of everyone affected by MS. Background Statement The Mid America Chapter began in 1955 as the Kansas City Chapter with one staff member working out of his home. By 1982, the Chapter had grown large enough to move to its current address and established the Ozark and St. Joseph branches. By 1985, the name was changed to the Mid America Chapter and in 1990, the Chapter merged with the Eastern Kansas Chapter. In 2010, the remaining counties in Kansas were consolidated into the Mid America Chapter. In 2011, the former Nebraska Chapter was incorporated into the Mid America Chapter. The Chapter, including the Kansas City, Springfield, Wichita and Omaha branches, currently serves 253 counties in Kansas, Nebraska and western Missouri, offers programs and services to over 50,000 individuals who are affected by MS and annually raises over $3.5 million to find a cure and achieve our vision of a world free of MS. Impact Statement The Mid-America Chapter works to improve the quality of life for people affected by MS in 253 counties in Kansas, Western Missouri, Nebraska and Pottawattemie County and raise funds for critical MS research. MS affects more than 400,000 people in the U.S., and 2.5 million worldwide. Top accomplishments for the past year include fundraising $3.486M for the chapter which 91% is allocated to direct services and programs. To invest $871K in research to end MS, and re-organization of the chapter to streamline work flow and costs. Goals for the current year are to: 1. Reinforce and implement the re-organization of the chapter. This would create one service approach to events, programs, and services. To be an organization of maximizing resourcefulness. 2.Increase volunteer engagement by 5% and volunteer leaders by 7% 3.Continue to stabilize the financial resources of the chapter. 4. capture the impact of our programs by using outcomes measures and then to communicate this impact more clearly. 5.Increase awareness of the multiple Sclerosis and MS. Needs Statement Our most pressing needs are: 1.Define synergized processes for all event management throughout out region. 2.Additional volunteers to partner for committees, projects and initiatives. 3.Increased revenue to expand mission delivery. CEO/Executive Director/Board Chair Statement The National MS Society s vision is a World Free of MS. And the Mid America Chapter helps to achieve this by leveraging volunteers and resources throughout a wide geographic area. We serve the entire states of Kansas and Nebraska, the western half of Missouri, and one county in Iowa. In recent years the chapter has gone through significant transition, including the realignment of territory and people served, and restructuring staff and resources to more efficiently execute the work across the territory. Our constituents include people with MS, their families and friends, health care professionals, legislators, volunteers, walkers, cyclists, fundraisers, individual donors, corporate sponsors, and everyone who wants to do something about MS. The chapter is successfully adapting to the changes in structure and resources, and has carefully aligned all work with the strategic priorities of the National MS Society. These strategies which include a focus on leading global collaborative MS research, connecting people with MS to what they need to live their best lives, fully engaging volunteers, driving policy change, and growing resources through individual giving, special events, and corporate revenue. We also continue to dedicate time and attention to building our organizational culture that is results-driven, embracing and celebrating learning, diversity and inclusion. Service Categories 3

5 Programs Programs Care Management Description Category Population Served Short-Term Success Long- Term Success Program Success Monitored By Examples of Program Success Care Management is a system by which the Chapter provides intensive and individualized care for people with MS. On-site social work professionals help clients with issues necessitated by their multiple sclerosis, such as help with housing, transportation, access to medical care, assistance with insurance and Social Security issues and family support. Under Care Management, direct assistance is provided to people with MS through financial assistance in a crisis related to their MS, or through medical equipment to help them maintain mobility and independence. Clients must complete an application and/or staff assessment to receive these services, and financial assistance is always provided to a third-party vendor. Medical equipment is dispensed either on a short- or long-term loan basis, through the Chapter s partnerships and collaborations with community vendors. Health Care Patient & Family Support Disabled, General or Disability Unspecified, General Public/Unspecified, Physically Disabled nec Short term success for this program will be demonstrated by recipients reporting: successful Social Security applications; successful resolutions in the areas of housing, transportation, and medical care; appropriate care and support for families; overall mental health and successful coping mechanisms; and ongoing independent living. As a result of Care Management, people with MS will live as independently as possible, and with the dignity of self-sufficiency. People with MS do not wish their lives to be about their multiple sclerosis, so as care management helps them manage the hurdles and crises of their disease, they can go about the other matters that are important to them, such as employment, parenting, and community involvement. This program was established in 2006 as a direct result from the feedback we received from clients and under the direction of our Board of Directors. It is through constant client feedback that we can assess the program strengths and weaknesses. This program is not a one-size-fits all model. Our outcomes are measured on an individual basis. We are currently in the process of formulating a formal outcomes measurement evaluation. Since inception the program has served more than 500 people living with MS, including securing accessible and affordable housing for several. Our social work professionals field approximately 200 calls per month, providing individualized service to each client, with several successes in the areas of Social Security, access to community resources and support, and overall mental health. Our Chapter is working alongside 50+ other MS Society Chapters throughout the United States under the direction of the National Office to develop a formal outcomes measurement plan that collects/reports both individualized and global outcomes data. 5

6 Long-Term Care Description Category The MS Society co-sponsored two accessible housing apartment complexes (One in Topeka and one in Kansas City, Kansas) in collaboration with Accessible Space, Inc. to ensure affordable and accessible housing in our community. These complexes have 20 living units that are primarily funded through HUD Section 811. These apartments are designed for people with MS or with another qualifying disability, and they are also based on income eligibility. The MS Society provides care management services at both apartment complexes. Our goal is to develop additional long term care projects and partnerships to support people with more progressive MS who are unable to stay in their own homes. Long-term success would be providing affordable, accessible housing options for all people in need with multiple sclerosis. We continue to have communication with the residents of the apartment complexes through our Care Management program. In addition, we have a long term care committee who meets regularly to address current needs and possibilities in this arena. We successfully opened two accessible apartment complexes in the Fall of We offer housing in 20 living units to people with MS or other qualifying disability. 6

7 Self-Help Groups Description Self-help groups bring together people who share a common life experience, for support, education, and mutual aid. Members of self-help groups share a belief that positive personal change happens through individual efforts with the support of others. The benefits of self-help groups to their members include: Learning new information and strategies for confronting problems. Finding reassurance and support from others with similar life experiences. Developing personal insights and achieving personal growth. The opportunity to be helpful to others. Feeling less isolated. Learning to express feelings better and to be more sensitive to the feelings of others. Feeling empowered and more self-confident in coping with challenges. Developing a connection to a community and personal identification with a social group. Long- Term Success Program Success Monitored By Examples of Program Success Individuals with Multiple Sclerosis have access to a self help group where they can receive emotional support, identify solutions to challenges and learn about resources in their community so they can improve their quality of life. In 1998 the National MS Society convened the NMSS Self Help Group Task Force. The charge to the task force was to develop guidelines and recommendations that would strengthen affiliations between self-help groups and NMSS chapters and maximize the efficacy of the groups. The outcome of their work was the development of National MS Society Self- Help Group Standards, Self-Help Group Leader training materials, and a tele-training program that was implemented in early These outcomes underscore the importance the Society places on the role of self-help groups and on the need to nurture positive relationships between groups and NMSS chapters. The Mid America Chapter employs a program manager who maintains the self-help group support network throughout the chapter territory that reflects the NMSS standards for selfhelp groups. The program manager also provides support, orientation and training for group leaders, distributes a quarterly newsletter. The Mid America Chapter has 24 self help groups spread throughout the market area; 10 in the Kansas City Metro area. Leaders from all metro groups attend at least one training annually and receive monthly newsletters containing information on upcoming events, resources for members and tips for successful meetings. All groups hold monthly meetings and average a minimum of 8 people at meetings. 7

8 Advocacy Description Category Our advocacy efforts are intended to increase awareness of MS and the MS Society among members of the legislative and executive branch in Kansas and Missouri. Our goal is to garner attention and support for bills and programs that promote access to services and increase quality of life for people with MS. Health Care, General/Other Health Care Issues Population Served Other Health/Disability,, Short-Term Success Long- Term Success Program Success Monitored By Examples of Program Success Our objectives are to increase volunteer engagement on our state advocacy committees, in action alert and in other advocacy initiatives, to identify key issues/bills and legislators on which to focus our efforts and to develop and communicate our targeted legislative agenda. Our long term success would be achievement in the areas of the National MS Society's Healthcare Reform principles: accessible, affordable, comprehensive quality health care services and coverage for all, access to high-quality long-term supports and services, standards for coverage of specific treatments, increased value of health care and elimination of disparities in care. We measure our success by the awareness of MS and the MS Society and the support received from various legislators as well as ultimately the status of the issues/bills for which we have advocated. An example from our Missouri advocacy efforts is our successful passage of the donation checkbox to benefit MS on Missouri state income tax returns. We collaborated with several other agencies to have a donation checkbox placed on Missouri state returns. Not only does this option increase awareness but raises additional funds as well. 8

9 Scholarship Program Description Category This program provides scholarships for college bound students who have MS or have a parent with MS. As part of their application process, they write an essay describing the impact on their lives. Often, a scholarship and the opportunities it represents are life-changing to these applicants. In partnership with ACT, we solicit and screen applicants and award scholarships annually. In the seven year history of this program, we have found that our scholarship recipients often come back to volunteer or intern with us, serve on the scholarship review committee and help us connect with their college/university and engage additional student volunteers. Education, General/Other Student Financial Aid Population Served Other Named Groups,, Short-Term Success Long- Term Success Program Success Monitored By Examples of Program Success The Mid America Chapter s goal for this program is that students succeed in college with reduced financial burdens as a result of the support of the MS community. The short-term measure of this goal is that students complete their freshman year of college and continue a second year of college. The long-term success is measured by evaluating students completion of their intended degree. Additionally, the Society evaluates the continued level of engagement by the student with MS Society. The Mid America Chapter distributes a survey to awarded students at the beginning of the academic year to assess who is continuing with their education and evaluate the level of continued engagement with the MS Society. Current scholarship recipients are continuing engagement with the MS Society by leading Walk teams for their families and fraternities. CEO Comments We continue to provide a broad array of programs and services to people affected by MS along the disease continuum from those newly diagnosed to those with more progressive MS. We also provide programs and services not only to those with MS but their family members and care partners. We continue to be recognized by our National office for providing high quality programs and services. We are also increasingly focusing on advocating to Missouri, Kansas and Nebraska legislators on issues of importance to people with MS. One of the biggest challenges is delivering this broad spectrum of programs and services throughout our very large geographic service area. 9

10 Leadership & Staff Executive Director/CEO Executive Director Term Start Aug Ms. Tami Greenberg Experience Tami Greenberg was selected to be the Mid America Chapter President in 2014, after the former Chapter President, Kay Julian, was promoted to Executive Vice President of the Northeast Region of the National MS Society. Tami had a successful career within the National MS Society from 2002 to 2012, beginning as the Vice President of Programs with the Mid America Chapter, and subsequently serving in a national role as Associate Vice President of Organizational Development and Interim Management. In 2012 Tami joined the United Way of Greater Kansas City team, as the Senior Vice President of Strategy Development and Human Capital. After 2½ years with United Way Tami returned to the National MS Society as Chapter President. Her prior career experience includes social work roles with Good Samaritan Project, The Children s Place, and The CASA Project. Tami has a Bachelors in Psychology from Wheaton College in Illinois, and a Masters in Social Work from the University of Kansas. She is a 2007 graduate of the Centurions leadership development program, and served for 2 years on the Alumni Board. In 2013 she completed the Kansas City Tomorrow program of the Civic Council of Greater Kansas City, and also served as a member of the inaugural Mayor s Challenge Cabinet in Kansas City Missouri, including chairing the Resources subcommittee. After leaving United Way of Greater Kansas City in a staff role she began serving as a volunteer on their Health Impact Council. Tami was profiled in the Kansas City Star cover story Giving Thanks in 40 Ways, and was recognized as a 2015 Influential Woman by Kansas City Business magazine. No Organization Comments Like many organizations, the Mid America Chapter felt the impact of the economic downturn and experienced significant declines in revenue in FY2014. We reduced expenses significantly while staying invested in our core mission related and operational areas. We did implement a reorganization to help synergize work efforts specifically in the development departments to centralize processes in event management. The restructure did come with employee alignment in job duties and responsibilities.
