2 Statements & Search Criteria Mission The National Kidney Foundation (NKF), a major voluntary nonprofit health organization, is dedicated to preventing kidney and urinary tract diseases, improving the health and well-being of individuals and families affected by kidney disease and increasing the availability of all organs for transplantation. Our Goals Supporting Research and Research Training Continuing Education of Health Care Professionals Expanding Patient Services and Community Resources Educating the Public Shaping Health Policy Fund Raising The National Kidney Foundation started in 1950 with Ada and Harry DeBold fighting for a cure to save their young son from pediatric nephrosis. Through Ada's determination, she was able to bring together a group of clinicians and researchers from around the United States to share their findings and develop new approaches to understanding and treating the disease, paving the way for The National Kidney Foundation we know today. The National Kidney Foundation is the leading organization in the United States dedicated to the awareness, prevention, and treatment of kidney disease. For more than 60 years, the National Kidney Foundation's efforts have helped benefit healthcare professionals, patients, and millions of Americans that are currently at risk. Through the NKF's development of the largest and most effective detection program, KEEP Healthy was established to help detect the earliest stage possible of kidney disease. KEEP Healthy has helped support kidney disease detection in 100,000 plus Americans since The National Kidney Foundation strives to promote kidney health through programs that emphasize awareness, prevention and treatment. These programs include: NKF Kidney Disease Outcomes Quality Initiative (KDOQI ) Kidney Disease: Improving Global Outcomes (KDIGO ) Kidney Cars "World Kidney Day" National Kidney Walks Impact NKF Accomplishments: KEEP Healthy The NKF has had over 60 years of service to people with kidney disease, their families, and healthcare professionals. Provided free health screenings to over 50,000 across the U.S. through our KEEP Healthy program. In the past year alone, the NKFVA has screened nearly 800 people - more than half of whom learned for the first time they may have chronic kidney disease (CKD). We have been recognized by the World Health Organization as one of the leading organizations in combating kidney disease. In Virginia, over 10,000 patients are on dialysis or waiting for a life-saving kidney transplant on a national list that is nearing 100,000. Every 5 minutes someone is diagnosed with kidney disease, the seventh leading cause of death in the US. Our goal is to lower these numbers through education, advocacy, and early detection. The NKF is ranked in the top 1% of socially responsible brands and meets the standards of the Wise Giving Alliance of the Better Business Bureau where 80% of every dollar raised goes directly to programs and services. Also, the NKF has a 4-star rating by Charity Navigator. 2

3 Needs Corporate Partnerships: Partnerships with local corporations help to raise funds and awareness to battle kidney disease, while building mutually beneficial relationships. The Foundation achieves life-saving results through the contributions of a generous public including participation in Foundation signature events, including Kidney Walks and the Cadillac Golf Classic. Board Members: The Foundation is seeking dedicated volunteers throughout Virginia who are well-connected in their community and are willing to open doors to assist staff in furthering the missions of the National Kidney Foundation. Special Event Committee Members & Active Volunteers: Serve and support patients and families whose lives were changed forever by a kidney disease diagnosis through considering joining a special event committee (Walk/Golf). The Foundation is always seeking special volunteers to hold fundraisers or percent-of-sales drives as well. Medical and Lay Programs Volunteers: Prevent or delay kidney disease from turning into kidney failure through free, community-based KEEP Healthy program. Areas Of Service Areas Served 3

6 Programs Programs Keep Healthy Description KEEP Healthy is the National Kidney Foundation's community-based initiative to educate about the kidneys, risk factors for kidney disease, and steps to take to keep kidneys healthy and reduce risk. It is designed to have the greatest impact on reaching the 73 million Americans that are at risk due to rapidly rising rates of diabetes, high blood pressure and obesity. KEEP Healthy raises awareness, prevents kidney disease and ultimately drives health care costs down. The program consists of a minute check-up where each participant receives test results, educational materials and information on next steps to take to stay healthy. A KEEP Healthy check-up is provided by a team of trained NKF volunteers. Budget $0.00 Strategy Yes Kidney Disease Outcomes Quality Initiative (KDOQI) Description The KDOQI education committee works to develop educational materials and programs designed to support and enhance implementation of KDOQI and KDIGO guideline recommendations, including printed and online materials such as GFR calculator, clinical action plans, and more, and educational conferences. World Kidney Day is an international day of awareness, with the NKF heading up the U.S. effort. The major objectives of World Kidney Day are to raise public awareness about kidney disease, its risk factors, and educate those who are at risk about the importance of early detection. 7 Kidney Cars Description Donate your car today to help the fight against kidney disease. Kidney Cars currently accepts vehicles whether they run or not, as long as it has a title. Donations can include cars, boats, motorcycles, motor cross bikes, power sports equipment including ATVs, off-road motorcycles, and personal water craft such as Jet-Ski with a trailer hitch. Kidney Cars also accept farm and construction equipment including tractors with a serial number. Budget $0.00 7
