4 serves over 26,000 youth, adults, and seniors in over 100 affordable housing communities across the country. Better Tomorrows' purpose at Janie's Garden is to provide case management and supportive social services to low-income residents of affordable housing communities which focuses on four core goals: (1) Educational Success, (2) Economic Stability; (3) Healthy Lifestyles; and (4) Strong Communities. Better Tomorrows' ultimate goal at Janie's Garden is to provide its residents with the educational tools and resources required in order that they may lead happy, healthy, independent and productive lives. Statement from the Board Chair Statement from the CEO/Executive Director Better Tomorrows provides case management services and critical community programming to low-income adults, seniors, children and youth. Better Tomorrows staff are located in community rooms within the affordable housing developments, allowing for direct access to the populations we serve. Our proximity means that our staff are better able to provide convenient, cost-effective, and outcome-driven programming. Better Tomorrows recognizes the need to create safe environments for the communities we serve. With this in mind, Better Tomorrows uses a uniform approach of case management and community programming to maintain our high standards of service. INDIVIDUAL CASE MANAGEMENT Better Tomorrows case management is built upon three standard practices: social service intake, assessment, and ongoing support. These practices allow Better Tomorrows case management services to go beyond reactionary management and proactively work with residents to achieve definable mutually agreed upon objectives. When a resident first moves into a Better Tomorrows community, staff immediately set up an introductory meeting with the individual or family. During this initial meeting, Better Tomorrows helps identify not only the current often more basic needs of the individual or family, but also set attainable goals for the future. Goals may include pursuing higher education, obtaining livable-wage employment, or maintaining a healthy home. Depending on their goals, Better Tomorrows shares relevant information on local resources including transportation, childcare, and low or no-cost food. COMMUNITY PROGRAMMING Using annual community assessments, standardized surveys, and a personalized understanding of the community s needs, Better Tomorrows identifies strengths and weaknesses within the communities we serve. We then design programming across our core program areas educational success, healthy living, economic stability or community strengthening to address the specific needs within each community. Once a specific programmatic need is identified, Better Tomorrows staff research the resources in the local community to discern whether or not a similar program is already in existence. If a similar program already exists, Better Tomorrows will coordinate with the other program to bring those services directly to the residents. When similar programs do not exist, Better Tomorrows identifies steps to create and implement a successful program. Better Tomorrows continually tracks key metrics and outcomes of our services to ensure our programs are cost-effective and impactful. Areas Served FL- Sarasota National 4

6 PROGRAMS The Watts Academy at Janie's Garden Description The Watts Academy at Janie's Garden was created in order to close the academic achievement gap that exists for students from low income families in Janie's Garden and surrounding communities. The Watts Academy achieves its goals through the following ways: 1. Targeting children ages 4 and 14 who are about to enter the educational system or those students who are below grade level by providing them with ongoing homework assistance and tutoring. 2. Providing immersive homework and tutoring assistance to ensure students who are on track stay on track and to help students who are struggling with their studies. 3. Providing a summer enrichment program through which participants engage in educational and community activities during the summer months. Budget $15, Category Program Linked to Organizational Strategy Population Served Short Term Success Long Term Success Program Success Monitoring Program Success Examples Education, General/Other Tutorial Programs Children Only (5-14 years) Poor,Economically Disadvantaged,Indigent At-Risk Populations Students participating in The Watts Academy at Janie's Garden will show quantifiable academic improvement in reading and math. Program participants will also show consistent school attendance and a reduction in behavioral issues. Program participants will be on point in national reading and math standards by graduation. A quantifiable reduction in student drop our rate will shown as will the number of students successfully graduating from high school. Through the newly established relationships with Sarasota County Schools, the program will collect student test scores, attendance records, and behavior records to track and evaluate students academic gains. In addition, Janie's Garden uses a computer based case management system, Efforts To Outcome (ETO), where data will be entered and reports generated detailing the effect The Watts Academe has on the participants. Since its inception, The Watts Academy at Janie's Garden has successfully helped over 260 youth improvement their academic performance Comments Program Comments by Organization Janie's Garden -- as with all Better Tomorrows communities -- is committed to capturing, measuring and utilizing data in its programming in order to positively impact the residents and community members it serves. In order to achieve this goal, Janie's Garden utilizes the Efforts to Outcome (ETO) data management system. ETO has been specifically designed for Better Tomorrows in order to provide ongoing analysis of existing programs, program funding requirements and, ultimately, successfully delivering quantifiable outcomes as defined by BT's logic model. Most importantly, any program and/or project undertaken by Better Tomorrows must align with the 6
