2 Mission & Areas Served Statements Mission Statement Heartland RADAC is committed to serving those affected by the impact of substance abuse by facilitating access, engagement and retention through the treatment continuum of care and fostering professional partnerships to improve services. Background Statement Heartland RADAC began as a department within Bethany Medical Center that provided substance abuse assessments, and incorporated as a private not for profit corporation in January of Heartland RADAC s roots in a hospital setting provided a framework for understanding of the implications of this chronic disease as well as the financial burden it imposes on families, communities and systems as a result. Today, services are provided in 76 counties throughout Kansas and are available to all individuals regardless of income, however, through various grants and contracts Heartland RADAC typically serves: individuals at or below 200% federal poverty guidelines, pregnant women, women with children, IV drug users, individuals who are HIV positive, clients involved with the Department of Children and Families, Department of Corrections, and Community Corrections. During the past couple of years, Heartland RADAC has added new contracts and partnerships which have provided the opportunity to service individuals with their own private health insurance, in state prisons, state hospitals, emergency stabilization facilities and others. Since its inception, Heartland RADAC s role in the substance abuse service sector has evolved from solely providing substance abuse assessments and screening, to an organization that serves as a central point of entry, facilitating access to treatment services while also assisting individuals through care coordination and case management in their process of recovery. Impact Statement This past year has seen significant changes and transitions at Heartland RADAC. In December, we said goodbye to the founder and CEO, Dalyn Schmitt, as she retired from her position of 18 years. Under her leadership this agency grew from a staff of 6 providing substance abuse assessments in 5 counties, to an organization with 69 employees proving substance abuse assessments, treatment, care coordination in 76 counties throughout the State of Kansas. I have learned a great deal under Dalyn s leadership and am looking forward to furthering the work of this great organization in order to meet the needs of individuals and families who are impacted by substance abuse. CEO/Executive Director/Board Chair Statement It is with great excitement and humbleness that I have assumed leadership of such an outstanding organization. While I am familiar with the programs, services, and staff of Heartland RADAC, I look forward to spending the next year looking at this organization through a new lens. Heartland RADAC was founded on the belief that individuals, can and do, recover from substance abuse. We also recognize that the progressive nature of addiction does not lend itself to simple or effortless changes in behavior. Individuals and families dealing with an active addiction must navigate through complicated systems and funding sources, and may not know how to access treatment and other recovery services. Heartland RADAC has walked with individuals down their path to recovery for over 18 years. We recognize that there are many approaches to recovery and continually strive for a balance between clinical innovation and human connection in all of the services we provide. I have always been proud to be affiliated with Heartland RADAC and am looking forward to further our professional partnerships and services in the next years. 4 Programs Programs Solutions Recovery Care Coordination Description Category Population Served Short-Term Success Long- Term Success Program Success Monitored By Solutions Recovery Case Management services are offered to individuals that receive Temporary Assistance to Needy Families (TANF) cash assistance through the State of Kansas Department of Children and Families (DCF) when alcohol and/or drug use is either clinically indicated (by an assessment), or when their use is interfering with their ability to fulfill the requirements and goals of this program. These services are designed to address substance use, employment, and stability with children. Mental Health, Substance Abuse Programs, General/other Substance Abuse Counseling Alcohol, Drug, Substance Abusers, Poor,Economically Disadvantaged,Indigent, Similar to long term success, short-term success occurs when clients access substance abuse treatment or other recovery services, become employed, and maintain custody of their children. Of the 442 served, 81% engaged in recovery activities or services. The long-term success of this is that clients become employed, maintain abstinence and avoid placement of children in DCF custody. In the last grant year, these services were provided to 442 individuals and their families. Of those, 85% became employed or were completing work component activities, and 96% maintained custody of their children. Heartland RADAC staff document all client activities in Heartland RADAC s Outcome Tracking and Reporting database, and are measured by the following criteria: Within a quarter, clients access substance abuse treatment or 12 step community support group, obtain part time or full time employment, or activities that will prepare them for employment, and who maintain custody of their children. Reports are summarized, and reviewed with staff and state contractors for quality improvement. Examples of Program Success The outcomes for these services reflect the time period of April 1, 2015 through June 30, During this quarter, 248 individuals received case management services. Of those, 85% (211) became employed; 96% (238) maintained custody of their children; and 81% (200) accessed substance abuse treatment or recovery services in the community. 4

5 Multiple DUI Care Coordination Description DUI Care Coordinators work with individuals who have received three or more driving under the influence convictions. The Care Coordinators work as part of a multi-team (client, parole, and treatment programs), and provide an extra layer of accountability to ensure all parties are clear and informed about program requirements and follow through. The Care Coordinators ensure clients are following through with treatment requirements in formalized treatment settings as well as community based recovery settings, such as 12 step meetings, church, and other support groups. Recovery Coaches provide additional support to individuals and linkage to recovery services and activities. Financial assistance is also available on a limited basis to help eliminate barriers to recovery such as safe and sober housing, and reliable transportation. In the last fiscal year, 1,047 individuals were served in this program. Category Mental Health, Substance Abuse Programs, General/other Relapse Prevention/Transitional Substance Abuse Population Served Alcohol, Drug, Substance Abusers,, Short-Term Success Long- Term Success Program Success Monitored By Short-term success of this program is defined using the following outcomes: increased accountability through clear communication among stakeholders, criminal justice clients successfully completed post-release supervision and all clients engage in the treatment process. During the last grant year, 1,047 individuals received services through this grant. 100% of all individuals accessed substance abuse treatment or other recovery services in the community. 83% of all participants discharged successfully. The long-term success for this program occurs when clients successfully complete post release supervision, do not drink and drive, and ultimately abstain from using all alcohol and drugs. Program outcomes are documented on Heartland RADAC s Outcome Tracking and Reporting database, and are measured by the following criteria: increased accountability is measured by communication once a month between corrections staff, client, care coordinators and/or treatment providers. Criminal justice clients completion of post-release supervision is measured by the number of clients who complete monitoring successfully. Clients engagement in the treatment process is measured by the number of clients engaged in treatment or recovery services. Examples of Program Success The following information is reflected for DUI clients only during the time period of July 1, 2014 through June 30, In total 1,047 unduplicated clients received services, 100% of which had monthly communication on their behalf with PO and recovery team to enhance accountability. During this time period, 100% engaged in substance abuse treatment and/or recovery services in the community. Also during this time period, 575 individuals completed post release supervision. Of those who completed, 83% (479) were successful at the end of post release supervision. 5

6 Intensive Case Management Description Category Population Served Intensive Case Management (ICM) services are provided to individuals with co-occurring substance use disorder and mental health issues who also have a history of homelessness and/or admission to state hospitals due to the progression of their illness. The goal of ICM services is to reduce or prevent admissions to state hospitals, secure sober living environments, engage in recovery support services,and mental health services. Mental Health, Substance Abuse Programs, General/other Substance Abuse Crisis Intervention Homeless, Poor,Economically Disadvantaged,Indigent, Short-Term Success Short term success occurs when clients access substance abuse services, obtain safe and sober housing, engage in mental health services, and avoid involuntary placements in a state hospital. In the last quarter, 218 individuals received ICM services. Of those, 94% (201) accessed substance abuse treatment or recovery services in the community, 78% (166) obtained safe and sober housing in their community, and 79% (169) engaged in mental health services. Long- Term Success Long term success occurs when clients live in a safe and sober environment in the community and are not admitted to a state hospital. Since May 2012, 592 individuals have been received case management services through this grant. Of those, 491 (83%) were not admitted to a state hospital after services were initiated. Program Success Monitored By Heartland RADAC staff document all client activities in Heartland RADAC s Outcome Tracking and Reporting database,and collateral information is obtained through a memorandum of agreement with the State of Kansas Aging and Disability Services (KDADS). Examples of Program Success From April 1, 2015 through June 30, 2015, 218 individuals received ICM services. Of those, 94% (201) accessed substance abuse treatment or recovery services in the community; 79% (169) accessed mental health services; and 78% (166) secured safe and sober housing. 6

7 Clinical Substance Abuse Assessments Description Category Clinical substance abuse assessments are conducted to determine if a substance use disorder is present. Clinicians licensed by the State of Kansas BSRB administer the assessment and follow up with clients who are clinically indicated for treatment to assist them in choosing the most appropriate treatment program or resource. Mental Health, Substance Abuse Programs, General/other Assessment for Substance Abuse Population Served Alcohol, Drug, Substance Abusers,, Short-Term Success Heartland RADAC is required to meet specific time parameters for some clients to schedule and receive a clinical substance abuse assessment. During the past grant year, Heartland RADAC met these standards on all assessments. 84% were completed within 14 days, 11% were completed within 24 hours, and 6% were completed immediately due to an emergent need. Long- Term Success Individuals will learn if substance abuse treatment is clinically indicated, and when it is, provided assistance in locating an appropriate treatment program. In the last year, Heartland RADAC completed 6,360 clinical substance abuse assessments. Of those: 63% (4,036) were clinically in need of substance abuse treatment and were provided assistance in locating and accessing the appropriate program. 37% (2,324) were not clinically in need of treatment. Program Success Monitored By Access to Care standards are documented on Heartland RADAC s Outcome Tracking and Reporting database, and are measured by the following criteria: emergent assessments must be provided immediately based on IV drug use or severe medical condition; urgent assessments must be completed within 24 hours, and routine assessments must be completed within 14 days. All assessments are reviewed for quality assurance and to ensure all required time parameters are met. A monthly report is provided to all staff reflecting total numbers for each month of the fiscal year. 8 Recovery Coaching/Peer Mentoring Description Category Recovery Coaching/Peer Mentoring is provided in conjunction to case management and care coordination services. It is a non-clinical service provided by individuals who have a personal understanding of addiction and the complicated path of recovery. The primary goal of this service is to help clients become connected and engaged in the recovery community. Mental Health, Substance Abuse Programs, General/other Substance Abuse Population Served Alcohol, Drug, Substance Abusers,, Short-Term Success Anecdotally, we know that the Recovery Coaches play a key role in helping clients achieve success and engage in Recovery Services. Recovery Coaches meet clients at 12-step groups, and assist them in locating social activities that are healthy and promote recovery. Long- Term Success During the last year Heartland RADAC has significantly increased the number of Recovery Coaches and the programs they provide adjunct services to. Because of this, a clinical supervisor was added to provide oversight and continuity within services. Outcome measures are currently in development and will be added to the data tracking system upon completion. Examples of Program Success Anecdotally, we know that the Recovery Coaches play a key role in helping clients achieve success and engage in Recovery Services. Recovery Coaches meet clients at 12-step groups, and assist them in locating social activities that are healthy and promote recovery. CEO Comments Navigating and accessing the substance abuse system and recovery community is difficult at best for someone in crisis. Heartland RADAC s expertise in these areas provides valuable assistance to individuals and families at a time of crisis when they are not sure who to call or where to begin their journey of recovery. Sometimes individuals fall through the cracks while they are waiting to access treatment services due to conflicting work schedules, arrangements for children, or there no treatment currently available. Heartland RADAC s programs and services are based on client need, the severity of addition, and level of involvement needed to address the substance abuse issue. As an organization we continue to focus on access, engagement, and retention through the continuum of recovery services as well as care coordination and case management. In the next year we will continue to explore emerging treatment modalities such as Medication Assisted Treatment (MAT), and the further development of Drug Courts in legal systems. 8

9 Leadership & Staff Executive Director/CEO Executive Director Mr. Jason Hess Term Start Jan Experience Jason Hess has over 21 years of clinical and management experience in the behavioral health field with 18 years of experience at Heartland RADAC. Jason began his work with the organization as an Assessment Counselor, was promoted to Utilization and Peer Review Manager, was promoted again to Clinical Director, and most recently promoted to the Executive Director of this agency. Jason is licensed as a Clinical Addiction Counselor by the State of Kansas and holds a Master's Degree in Clinical Psychology from Emporia State University. Jason is recognized as an expert in this industry and has served on many statewide boards and coalitions. Kris has many years of computer experience and training and holds both a Bachelor and Master degree in the field of Human Ecology. She worked for several years as a Human Resource Professional and is responsible for directing the human resources activities for the organization. She has been employed by Heartland RADAC for two years 9

10 Ms. Kathie Panella-Crosby Title Experience/Biography Clinical Director of Treatment and Assessment Services Kathie Panella-Crosby is the Clinical Director of Assessment and Treatment Services for Heartland RADAC and has over 19 years of experience in the behavioral and physical health fields. Her previous work experience includes the Director of Social Work in the Behavioral Health and Acute Care Department of Truman Medical Center, and was previously the Psychiatric Consultation Coordination, and oversaw intensive outpatient programs for adults at that hospital as well. Prior to that Kathie provided clinical leadership the Behavioral Health Department of St. Paul Medical Center in Dallas Texas. She holds a Master s Degree in Social Work from University of Texas, Arlington, and a Bachelor s Degree in Science and Human Development from Texas Tech University in Lubbock TX. In addition to her work and educational experience she is licensed Clinical Social Worker in both Kansas and Missouri, a member of the National Association of Social Workers (NASW) and National Alliance of the Mentally Ill (NAMI). She is a certified instructor of Non-Violent Crisis Prevention Intervention (CPI), and has advocated on behalf of behavioral health needs for many years. Mrs. Peggy Taylor Title Experience/Biography Director of Financial Services Peggy Taylor is the Director of Financial Services for Heartland RADAC and has over 27 years of experience in accounting and business management for both for profit and non-profit organizations. Her previous work experience includes Accounting Manager for leading Energy Consulting/Management firm; Architect & Engineering firm; Accounting & Payroll Software Account Executive and Seattle Behavioral Health Agency. She holds a BA degree from Wichita State University. In addition to her work and educational experience she serves as Church Treasurer; Finance Director for Faith Based organization; recipient of Norman Justice Humanitarian Award for Community Service, Christian Business Women of the Year-KCK; and member of Delta Sigma Theta Sorority.
