2 Statements & Search Criteria Mission The Institute for Public Health Innovation (IPHI) creates partnerships across sectors and cultivates innovative solutions that improve health and well-being for all populations and communities across Virginia, the District of Columbia, and Maryland, particularly those most affected by health inequities. Background Across the country, public health institutes have a unique and important role in their states and communities, serving as cross-cutting non-profit resource organizations that work in collaboration with a broad range of partners to improve community health. Over 40 such institutes now exist across the country as part of the National Network of Public Health Institutes. IPHI is the official public health institute serving the Commonwealth of Virginia, and has an office in Richmond that serves as a base for supporting the entire state. IPHI has grown steadily since its establishment in 2009, particularly in Virginia. Our organization has consistently had an operating budget of over $3M/year and an average of roughly 30 staff over the past 4-5 years. IPHI now has approximately 35 funders, including federal, state, and local government; national and local foundations and grantmaking charities; corporations; health care providers; Medicaid managed care organizations; academic partners; and other supporters. The following are four broad categories that characterize IPHI s work: Community-Based Health Promotion IPHI partners with healthcare providers, community organizations, governmental agencies and others to design, implement and evaluate community-based approaches to public outreach, health education, and the overall improvement of accessible, quality health services. Training and Workforce Development IPHI supports public health professionals to enhance their technical skills, functional expertise, and leadership capabilities. Research for Action IPHI evaluates the effectiveness of public health programs and works with its partners on practical research that informs efforts to create healthier communities. The emphasis is on gaining information that strengthens the ability of organizations and communities to improve the public s health and reduce health inequities. Policy and Health Systems Improvement IPHI works with partners to pursue policy, systems and environmental changes that create healthier communities. We provide technical assistance to help develop and implement policy initiatives that promote healthy food access, active lifestyles, a healthy workforce and prosperous communities. Impact IPHI has firmly established itself as a regional public health institute and now has an operating budget of $3M and 33 staff. Through its Healthy Eating and Active Living (HEAL) Campaign, IPHI provides free technical assistance and training to help municipal leaders in municipalities in Virginia and Maryland adopt policies that improve their physical activity and food environments. Close to 50 cities, towns, and counties have joined the campaign by passing a resolution containing a HEAL policy agenda. Of those, 60% have passed subsequent policies. Policies have focused on streetscapes, healthy vending, transportation, food systems, and other areas. IPHI has trained over 600 community health workers (CHWs) in our region, and through our own projects have created over 30 jobs (mostly full-time) for CHWs. Enrolled over 1,200 in CHW-based services for chronic diseases. IPHI has established a partnership with the Virginia Department of Health to co-facilitate the statewide CHW Advisory Group. We are providing technical support for the creation of a statewide CHW Association, which is now fully functional. IPHI partners with VCU Department of Psychology on RVA Breathes, a NIH-funded research study on the impact of evidence-based strategies to improve outcomes among children with asthma. IPHI manages the community health worker portion of the project. IPHI partners with Health Quality Innovators on RVA Accountable Health Community, a CMS-funded study on the impact of aligned systems of care to screen and refer Medicare and Medicaid patients for services to address their needs related to housing, transportation, utilities, interpersonal violence, and food. IPHI manages the navigation portion of the grant through the utilization of CHWs and coordination of navigation services provided by partnering health systems. IPHI serves as the fiscal agent for the Virginia CHW Association. IPHI provides training and technical assistance to public health, medical, government, non-profit, business, and grassroots organizations on creating healthy and equitable communities by developing programs, policies, and services that are inclusive and equitable. IPHI provides training and technical assistance to local governments to assist them in adopting a whole-of-governmental health in all policies (HiAP) framework for decision-making across City departments in order to promote population health and health equity. 2

4 Programs Programs Healthy Eating Active Living Cities and Towns Campaign Description HEAL Cities and Towns provides free technical assistance to local elected officials to assist them in adopting ordinances and other policies that support their residents in increasing their physical activity, increasing their consumption of healthy foods and to support municipal employees in being healthy. Budget $0.00 Community Health Workers Description IPHI provides regional leadership in expanding the community health workforce. CHWs are trained community members with a trusting and often unique relationship with the community served. They often serve as links between community residents and the health care system. IPHI has established a regional Center for the Community Health Workforce to support the growth of this important aspect of the public health and health care workforce. IPHI develops and provides training for CHWs, their supervisors and health systems interested in utilizing them. IPHI also assists partners in developing models of care and health promotion that incorporate CHWs. IPHI is working with VDH to develop standards for CHWs, including scope of practice, competencies, training opportunities, and sustainability. Budget $0.00 Community Health Assessment and Health Improvement Planning (CHA-CHIP) Description IPHI supports communities in conducting community health (needs) assessments and creating evidence-based community health improvement plans. We also serve as a backbone organization to assure effective implementation of those plans with diverse stakeholders. Our work is based on the principles of evidence-based public health, health equity, community engagement and social determinants of health. Budget $0.00 Health in All Policies Description Health in All Policies (HiAP) is defined as a collaborative approach to improving the health of all people by incorporating health considerations into decision-making across sectors and policy areas. HiAP focuses on community health, health equity and sustainability as important and intersecting aspects of decision-making. IPHI supports partners in utilizing this framework to achieve greater impact through improved collaboration, efficiency, equity and effectiveness. IPHI works with individual organizations, as well as with local governments to develop allgovernmental HiAP policies and strategies. Budget $0.00 4
5 Training on Health Equity and Social Determinants of Health Description IPHI provides training and workshops to organizations, communities, coalitions and others on how to effectively advance health equity and social determinants of health. Our curriculum increases awareness of health inequities, social determinants of health, the impact of systems of discrimination on health, community-based participatory approaches and evidence-based/informed strategies to promote health equity. We also provide in-depth technical assistance and capacity building for organizations that are interested in reassessing their organizational policies, practices, programs and services to assure they are all aligned with their commitment to health equity. 8 Management Executive Director/CEO Executive Director Term Start Jan Michael E. Rhein MPA Experience Michael has worked on the local, regional and national levels to improve the public s health, with a particular focus on providing strategic support to communities so they can strengthen their service systems and develop effective policy and programs to eliminate health inequities. Throughout his career, he has served in intermediary roles, helping to translate public and private resources into practical support for communities. This has involved collaborating with the Federal Government, large private foundations such as W.K. Kellogg Foundation and Ford Foundation, national corporations, state and local health departments, local foundations, academia and a host of other partners across the country.michael helped launch IPHI beginning in 2009, moving into the role of its first full-time President & CEO in January Prior to joining IPHI, Michael served for nine years with the National AIDS Fund (NAF), most recently as Vice President of Programs. During his tenure, NAF s annual grantmaking to communities nearly quadrupled to over $7 million with the launch of multiple major national initiatives to address key challenges in the modern HIV/AIDS epidemic. From , Michael managed grantmaking and technical assistance activities for W.K. Kellogg Foundation s National Program Office of the Turning Point: Collaborating for a New Century in Public Health initiative, based at NACCHO. Turning Point included 41 diverse local sites from New York City to communities in Alaska engaged in collaborative strategic planning and action to create more responsive, community-based public health systems. Michael has served on several local nonprofit boards in the Washington area and was selected for the inaugural class of the Non-Profit Roundtable of Greater Washington s Future Executive Directors Fellowship Program in He holds a Master s degree in Public Administration from the University of Southern California and a Bachelor s degree from The George Washington University.
