2 Statements & Search Criteria Mission Empowering cancer survivors through accessible yoga and mindfulness practices. I have experienced and witnessed a large gap in the healthcare system. The mind /body connection is often overlooked. Beyond medical treatment, the emotional/mental healing is a large and sometimes unexpected piece to becoming whole after a cancer diagnosis. Survive & Thrive want to empower and inspire survivors to o tap into the part of everyone that is well. 3 Programs Programs Community Classes for Cancer Survivors Description Long-Term Success Examples of Long-Term Success Target Population 1 Building the local community and network of support. Wendy & Tracey will offer free weekly classes. and monthly workshops.these will be based on our personal experience dealing with cancer, research, and our continuing education. Although cancer is the leading cause of death in Collier county, people are surviving longer due to advances in wester medicine. We believe that cancer is a word, not a sentence. We want to empower those that have ever been diagnosed to live a full and joyful life. We want to enhance the quality of life of not only survivors, but their families the comunity and beyond. For example, participant has decreased a anxiety, physical pain, increased range of motion and joy. The participant will list their the main issues they are experiencing and we will check back in those after a certain amount of sessions. We have created metrics that will track individuals. People/Families of People with Cancer Target Population 2 Individualized Wellness Curriculum Description Long-Term Success Examples of Long-Term Success Target Population 1 A cancer survivor will be funded for a specified amount of time. The curriculum will include; yoga & meditation to meet the needs of the individual. The participant will practice weaving these skills into relationships, food, work, all aspects of life. Each participant will encouraged and support to spread their knowledge and experience in the Survive & Thrive community. There is an option to become a certified yoga teacher through Center Point Yoga School. This program will grow the web of support to make these service available to more survivors. This program could be implemented in cancer care facilities, healthcare providers could be trained to offer this program in group settings. We have created metrics to survey entrance data and exit surveys. Testimonials and long tern follow up will be a key component tracking program success. 4 Video library Description Long-Term Success Examples of Long-Term Success Target Population 1 Target Population 2 Survive & Thrive will create an extensive and accessible yoga/meditation/breathing techniques video library specifically designed for cancer survivors. This project will be ongoing, as funding allows, the more videos we can produce. The idea is have a practice for a survivor that may barley be able to move to a survivor that is ready for more physical activity. The videos will be on the Survive & Thrive U tube channel, this channel can be accessible in cancer treatment facilities. They will be available on The Survive & Thrive web page. This project has potential to expand to kiosks in public and private healthcare locations, truly bridging the "missing peace in healthcare". Bringing awareness to the emotionally isolating, devestaing and scary journey of a cancer diagnosis. Empower cancer survivors, families and healthcare workers to not just treat the physical body, but tapping into the potential peace and joy of the present moment. Making these videos available could provide the emotional support, guideance, and structure for survivors to make the most out of breath that left Those using the videos will fill out a brief survey including; physical/emotional symptoms. These surveys will help us as we add to library. After a certain amount of time a practice we the participant will fill out another survey. 5 Impact Goals What are your organization's long-term strategic goals? Survive & Thrive's mission is to empower those living with, through and beyond cancer by bringing awareness to the gap between external medicine and internal alchemy using yoga, meditation and community. We are especially sensitive to the needs of the uninsured and to those living in underserved communities, and it s our intention to reach out to these publics. We will also be a knowledge base and resource to healthcare providers and individuals that serve patients in cancer treatment centers, hospice, recovery groups and more by providing videos, weekly local classes, and onsite workshops and training. They have directly experienced healing and have found purpose and renewal through their yoga practices. There seems to be lack of awareness between mind/body in most healthcare systems. We will help to bridge that gap. Studies show that a mindfulness practice can change our DNA. Survive & Thrive is deeply connected to bringing the compassion piece to cancer survivors. Facing a cancer diagnosis and treatment can leave the soul numb and send the survivor into a constant state of anxiety and fear. We are passionate about sharing and empowering cancer survivors to connect with their innate well being and healing that lives in ALL of us. When we are connected to our spirit, we gain access to hope. Dignity, strength and joy can be accessible at every stage of the healing process. Cancer is a word, not a sentence. Strategies What are the major strategies you will do to accomplish your goals? In order to promote a community of survivorship that is empowered and connected, Survive & Thrive will: Do continued research and training to develop programs that best suit the needs of cancer survivors.establish a base of support including donors, corporate donors and services and space from local businesses that want to help. Apply for grants and implement fundraisers for video equipment, technical and yoga gear, advertising, wellness program sponsorship, workshops/retreats, etc.continue community outreach in healthcare facilities, churches, and social media. Capabilities What are your organization s capabilities for doing this? Both Wendy Campbell and Tracey Eugland are cancer survivors and yoga teachers, and have experienced the healing benefits of a yoga/mindfulness practices. This organization was born out their direct experiences of the missing piece in healthcare. The Survive & Thrive board is diverse, with everyone bringing a unique skill set. We have people deeply connected into the community, healthcare professionals, experienced board members, and most of heart. Indicators What indicators do you use to measure progress? We have created metrics and qualitative and quantitative data for our programs and impact worksheets that will track individuals, groups and the web based survivor community. During the first year we will collect date from at least 25 survivors. We will also collect testimonials from participants. This process will be expanded upon each year and the data collected will inform our programs as we move forward. Progress What have and haven t you accomplished so far? Survive & Thrive has established a board, developed bylaws, and held several meetings including 2 board meetings. We have completed the paperwork to be an an official 501(3c). We have a live webpage, logo and social media accounts. We are in the process of applying for our first grant with The Pollination Project. We are 5

6 reaching out into the community through a newsletter to fund our first project. The Naples Daily news is featuring our organization this month.we are scheduled to speak at the Women's Cultural Alliance in February. We will be offering our first workshop at Shangri La in the next month. A November 2016 article in Psychology Today has been published 6

8 Management Executive Director/CEO Executive Director Wendy Campbell Term Start Oct Experience In 2006, I was diagnosed with Hodgkins Lymphoma and everything changed including my relationship to yoga. I had a very clear moment when I understood that yoga is something I would do to some capacity for the rest of my life. My casual yoga practice evolved into mental/emotional life line. After finding my footing, I ended my career in the brokerage industry to study, practice, and teach yoga full time. I am inspired to help others that have been affected by cancer and other challenges that one will survive in a lifetime. I have worked extensively in the Naples community teaching classes, privates, and operate and teach Center Point Yoga School, certifying yoga instructors through The National Yoga Alliance. By sharing the knowledge that I have learned and wisdom I have gained through practice and way of life, I believe there is always an option to step into greater alignment and thrive. I have worked with several non profits in the past including; Save the Children as an AmeriCorps Member, Facilitating people with disabilities on multi day river trips for SPLORE and Volunteer Coordinator for over 200 volunteers, and Life Adventure Camp.
