Infinite Wishes Foundation is in memory of my best friend, Donna's sister-in-law and Al's sister Joan, who developed stage 4 lung cancer at the age of 53. Since she was a young girl, it was Joan's dream to swim with the dolphins. She loved dolphins and anyone that knew her, knew that, too! Joan was diagnosed with stage 4 lung cancer in February Joan had plans to come to Florida and I wanted Joan to have her wish fulfilled. I began researching for foundations to help grant her wish. As I researched, I was shocked to find there were only a few organizations to help adults and they were either state specific, disease specific, or had criteria she did not meet. I began reaching out to businesses in Florida to help with our desire to fulfill the wish for Joan. Reaching out to area businesses, I was able to get a hotel stay and Discovery Cove donated a day for Joan's to swim with a dolphin. That day, Joan looked and me and said "This is the best day of my life". Those words touched my heart and forever changed me. While Joan was in Florida, I also reached out to other local businesses to make her trip here memorable. Many businesses donated their services for Joan. The outpouring of support made me realize how much good there is in this world and I thought to myself "What could I do for people like Joan if I had money?" Joan died December 20, 2013, and in her memory, Infinite Wishes Foundation was established to help bring a diversion and some joy into the lives of other adults facing life-threatening illnesses. Dozens of organizations exist to help children with life-threatening illnesses fulfill their dreams and wishes. However, there are few that provide the same for adults. The goal of Infinite Wishes Foundation is to give those same opportunities to adults offering them a renewed sense of strength and hope. Having had personal experience with observing the impact of granting Joan's wish, I saw first-hand what giving someone a chance to fulfill their wish can do to change the overall outlook and add some relief from the battle against their disease. Joan came to Florida after 15 rounds of brain radiation. The stress, strain and effects of the treatments took a toll physically and mentally. When she had the opportunity to take a break from her illness and treatments; the results were overwhelming to see. It was as though for just a little while she was cancer-free and the worry and burden were lifted. She made an almost miraculous turnaround and was able to sleep, eat, and enjoy life as before her diagnosis. I bear witness to what the effect of time away, in a different environment; void of the treatment routine did for my friend. With a little bit of effort on my part; I was able to make a difference in her life. She went back home stronger, with a more positive outlook, rested and relaxed; to begin the next phase of treatment - chemotherapy. Months of research on the aging population and the impact of wish granting made me realize that this organization fills an under-served population of adults who have a life-threatening illness; who are just big kids deep inside and wishes, too! I am convinced that most people do not grow up...we marry and dare to have children and call that growing up. I think what we do is mostly grow old. We carry accumulation of years in our bodies, and on our faces, but generally our real selves, the children inside, are innocent and shy as magnolias. Maya Angelou The 3

4 foundation's philosophy is simple: working together to deliver dreams; providing lasting, memorable experiences and enhancing the lives of adults with a life-threatening illness. From the Washington Post: There were 76 million people born between the years 1946 and 1964, the traditional window for the baby boom generation. That means that they will retire over a 19-year period. Simple math shows that 76 divided by 19 is 4 million, or almost 11,000 people a day. From The Center to Advance Palliative Care Facts and Stats:Approximately 90 million Americans are living with serious life-threatening illness, and the number is expected to more than double over the next 25 years with the aging baby boomers. By 2020, the number of people living with at least one chronic illness will increase to 157 million. Today, seven out of ten Americans die from chronic disease Statement from the Board Chair I am very honored to be part of Infinite Wishes Foundation. Joan was not only my husband's sister, she too was my sister; forever in my heart. Her death left a profound silence in our hearts, but her life as she lived it gave us the inspiration to help others. The challenges we face as a nonprofit is the same as other charities; operating on a limited budget, with limited resources, working diligently to raise awareness and funding to carry out our mission. Our commitment to Joan's memory keeps us moving forward.we have grown by leaps and bounds these past few months -- almost too good to be true. We are forever grateful to our guiding star and angel...joan; who is the inspiration in helping others in need. May she forever be present in our desire to help others gather strength and courage while creating happy memories. 6 Infinite Families Description In Memory of our both my mother and Donna's mother, we have a program to help children 18 years of age or younger, with a parent battling a life-threatening illness, and the child themselves have been diagnosed with a life threatening illness. We have partnered with Children's Wish Foundation International to fulfill wishes for Florida families the have an adult with a life-threatening illness and also have a child with a lifethreatening illness. We will collaborate efforts to fulfill wishes for both the adult and child in memory of Joan and children in memory of our mother's. Our mothers were very active and supportive of children's charities. This program carries on their legacy and their love for helping children with a life-threatening illness. The long term success will be to touch the lives of both the children and the parents with a life-threatening illness a more enhanced experience by offering to include the child's wish that can be combined along with the adult wish that we will fulfill for the adult. The future program will be measured by the feedback from the parents, children and family as a whole. Follow-up phone communication will be recorded and reviewed to look for ways to monitor and expand the program. We will continue to develop new ways of creating a more memorable family experience. Our future program and the supporting data compiled and reviewed by the board to measure the program's success and evaluate its impact. 6
