2 Mission & Areas Served Statements Mission Our mission is to provide a healthy, safe, nurturing and culturally welcoming environment that promotes a partnership of trust between the individual in recovery and the agency staff. Our treatment approach respects the culture of each person, welcomes family involvement, responds to the unique recovery needs of women and of men, and strives to create a climate that promotes safety, trustworthiness, choice, collaboration and empowerment. 2

3 Background In 1971, Crossroads was founded to respond to the unmet needs of the Latino population and began providing addiction treatment services in 1973 for men, began a 10-bed program for women in 1977, and began serving individuals referred by the Department of Correction in 1987; our organization fulfilled the original mission by establishing a bilingual, residential substance use treatment program to serve adults throughout the State of Connecticut and then extended our mission to respond to the needs of a culturally diverse population. As the needs of the population served changed, the organization expanded our addiction services to both residential and outpatient levels of care, initiated outpatient mental health services for persons with co-occurring disorders, and collaborated with the Cornell Scott Hill Health Center to provide on-site primary medical care. In 1989, Crossroads moved from 42 Howe Street, where it had begun, to a larger facility at 54 East Ramsdell Street which offered room to grow. Gender-responsive services were expanded by 8 beds for pregnant and parenting women and parenting education and support were provided by The Consultation Center. Collaborating with the Office of Alternative Sanctions, now the Court Support Services Division of the State of Connecticut, and with the United States Probation/Pretrial Services Office, capacity was increased to 110 beds in this residential facility in a 44-bed Women's Unit and a 66-bed Men's Unit. By 1994, the new facility had almost reached capacity when Crossroads was awarded a CSAT grant to open a specialized women's and children's program, Amethyst House, with 15-beds for women and 15 for their infants/children; Crossroads returned to lease space for this program at 42 Howe Street. In 2014, we celebrated the 20th anniversary of Amethyst House. In 2003, Crossroads sought and achieved accreditation from CARF, the Commission on the Accreditation of Residential Facilities, and has maintained that accreditation by implementing the standards of CARF to deliver personcentered, quality behavioral health services. In 2006, the Outpatient Behavioral Health Unit became operational at 44 East Ramsdell Street, providing intensive outpatient and outpatient addiction treatment and outpatient mental health services. Over the years, clinicians providing those outpatient mental health services have been increased to the current contractual staffing of two part-time, bilingual psychiatrists who provide psychotropic medication evaluation and management, and four part-time therapists who provide mental health intake assessments, individual/family/couple's psychotherapy and group psychotherapy to address anger management or domestic violence. In 2008, Crossroads received funding through the support of the CT Latino Behavioral Health System to expand services for monolingual Spanish-speaking individuals receiving outpatient services and currently the organization sustains that additional position. In 2009, the Cornell Scott Hill Health Center initiated a medical clinic in the outpatient facility of Crossroads to provide primary care services to persons served in the residential programs. In 2010, the Recovery Campus of Crossroads at East Ramsdell Street was completed with the construction of a facility intentionally designed to better meet the needs of women and children; a 2-story annex to the original facility at 54 East Ramsdell Street now consolidates all women's services; Amethyst House was relocated to this facility, with capacity for 18 pregnant and parenting women and up to 26 infants/children. A wing of this new building was designed for licensed day care; Amethyst House collaborates with LULAC Head Start, who operates a licensed Child Day Care Center, Little Gems, in a separate wing of the residential facility, to serve the children of women in Amethyst House and children from the local community. The original treatment facility now exclusively serves the Men's Residential program which has grown to an 89-bed unit. Over the last 43 years, beginning with Miguel Laguna, one of the founders and the first Executive Director, and continuing with later leadership, the organization has faithfully implemented our mission and expanded our services to better respond to the changing needs of the persons served. Today our mission is to provide a healthy, safe, nurturing and culturally competent recovery environment that promotes a therapeutic alliance with the persons served and fosters the self-empowerment of those recovering from addiction and/or mental health disorders. To prepare to respond to the needs of those served, the organization participates in practice improvement initiatives of the Department of Mental Health and Addiction Services including the Women's Services Practice Improvement Collaborative (WSPIC), the Trauma-Informed and Gender-Responsive System of Care initiative, and the Co-Occurring Collaborative Impact Crossroads, Inc. has developed a relationship with Lulac Daycare facility to further the needs of Amethyst house Women and children. Crossroads has new upper management that will bring positive changes to the internal workings of client focused treatment. Crossroads has accomplished a growing intensive out-patient program. In the up coming year, Crossroads will continue to work along side Lulac and finish the playground area for the children. Crossroads will go live with Carelogics an electronic records program which will afford greater links to services provided, as well as increase billing accountability. Crossroads is currently completing a new roof. Crossroads is in connection with the Nature preserve and hopes to continue with the building of a public walk through for a more pleasing atmosphere to the population served. 3

CEO Statement Crossroads was founded in 1971 to serve the particular needs of Latinos and other minorities affected by alcohol and illicit drug abuse and addiction. Since then the organization has expanded both in size and services to meet the needs of a diversity of clients and also treating co-occurring mental health disorders. Crossroads was among the first substance abuse treatment facilities in Connecticut to provide residential services to women and also women with minor children. The agency is licensed by the Department of Public Health (DPH) and funded by the Department of Mental Health and Addiction Services (DMHAS) and Court Support Services Division (CSSD). In keeping with its mission, Crossroads aims to provide a safe, nurturing and culturally affirming environment where recovery from the disease of alcohol and drug addiction can occur and be effective. Every client has a counselor and each client receives twenty hours of direct therapeutic intervention disseminated in group and individual sessions and talk therapy. Clients also have the opportunity to complete GED classes and participate in job-seeking programs. Crossroads collaborates with a wide range of locally based service providers such as dental clinics and the Cornel Scott Health Center, which provides on-site medical services and makes referrals for additional medical treatment. Crossroads has a number of volunteers who facilitate AA and NA meetings in-house as well as church volunteers who have religious services on-site or who transport clients to services and event in the community. The agency also offers intensive outpatient services (IOP) for our graduates as well as individual who apply for this level of care. Many of our clients come voluntarily for treatment, but Crossroads also caters to a large group of men and women who are involved with the legal justice system because of disease. The agency has a long list of graduates and their families who are grateful for the services they received while clients and for the opportunity it afforded them to avoid or lessen their incarceration period and redirecting their paths in positive ways. Crossroads has been a refuge and safe place for several women and children escaping domestic violence and abuse. Over these forty years Crossroads has seen many families reunited and the lives of men, women and changed. Crossroads seeks funders; partners and volunteers who believe in second chances for human beings affected by addition and are trying to escape the pain and destructiveness of their disease. Crossroads welcomes support from agencies and individuals who value and celebrate multiculturalism and diversity in all forms. Crossroads has created a niche in addiction and co-occurring treatment in Connecticut that is appreciated by many including clients, State agencies, and stake holders. This is an achievement that is worth preserving, but also one than needs to expanded and shared. Board Chair Statement As President I have a commitment to this organization that goes back 40 years, as one of the 3 individuals who opened the doors in Throughout this many years I have continued to work and support all the program and needs that have been part of its development. Recently, as President I have had to address a major challenge in terms of our management, and because we were facing very serious concerns from our funding agencies, we were forced to take steps in the change of the top management. We have met this challenge during this period of transition by taking over the management on a interim basis by a member of the Board who has extensive experience in the management of agencies like ours, as well as being a Professor of Social work, and myself; working on a transition in the process of hiring new management, and stabilizing the agency as a whole. Because we also find ourselves in a serious financial situation, which is important to point out, has never happened previously in the entire existence of Crossroads, I am leading a major fund raising campaign to help us create self-sufficiency to re-establish the strengths and vitality of our organization. 6 Programs Programs Crossroads Residential Program Description Crossroads is a residential rehabilitation program for 140 men and women who suffer from substance abuse. The programs provides long term care between 3-6 months affording people the opportunity to recover their lives as well as finding employment, housing and returning to the community and their families. It is a bilingual and bicultural program and recognizes the needs of individuals that come from different backgrounds with unique needs, such as a language barriers and cultural differences. Connecting the individual with community support systems is very important for the ability of the individual to have a smooth transition and regain stability. Transportation is provided for clients to attend their court, medical and therapeutic services. The goal is to have individuals remain substance free, not reenter the criminal justice system, for women to return as healthy members of their family and to be able to care for their children. Amethyst House Description Amethyst House is a program for pregnant women, and women residing with children in treatment. It is the largest program in the state of Connecticut serving up to 18 women and 26 children. We work towards reunification with DCF services. Budget $117, Category / Population Served / / 6

7 Intensive Out-Patient Program Description Outpatient services and intensive out-patient services offer culturally appropriate treatment help for both addiction and mental health. Services available are mental health evaluation, crisis intervention, group counseling, life skills development, military support program, (gulf veterans and their families), psychotherapy, family counseling, medical clinic and Phlebotomy services, (provided by Hill Health Center Satellite clinic at Crossroads, Inc. since 2008) Budget $395, Category / Population Served / / 7
