2 Statements & Search Criteria Statements Mission Statement For 30 years, children and youth who have experienced the worst in life have found respite and new hope for a better life through Specialized Alternatives for Families and Youth of Kentucky, Inc. (SAFY of KY). Our mission of "Preserving Families and Securing Futures" is achieved through a Model of Care that works with families across systems to enhance their capacity to care for and protect their children. Focusing on permanency, safety and well-being and needs within the context of the family and community, we build on individual's strengths to achieve positive, sustainable outcomes for families and children. Background Statement Since 1998, SAFY of Kentucky has provided hope and homes to youth in Kentucky. SAFY of KY specializes in providing therapeutic foster care to at-risk youth throughout the state. Therapeutic foster parents are specially trained and supported 24/7 by extensively trained, clinical staff to meet the unique needs of each youth and support their health in all domains. This model of care allows children to live in a family setting in their community rather than an institution. While working to find a permanent family through adoption or reunification, some youth age out of the foster care system and SAFY is dedicated to serving them into successful adulthood through its Independent Living program. SAFY of Kentucky is a Behavioral Health Services Organization (BHSO) and Seven Challenges provider offering outpatient, in-home and community-based mental health services including adult and family therapy, play and art therapy, screening and assessment, medication management in partnership with other facilities, Juvenile Sex Offender (JSO) assessment, Trauma-Focused Cognitive Behavior Therapy (TF-CBT) and substance abuse counseling services. In 2016, SAFY of KY served on average: 234 children per day in Treatment Foster Care; 31 youth per day received Independent Living services; 60 families per month received Family Preservation services; and 26 youth achieved permanency through adoption. Over the course of the year, SAFY of KY served 498 families and provided 175 children outpatient Behavioral Health services. SAFY of Kentucky benefits from the national presence and decades of experience of Specialized Alternatives for Families and Youth of America, Inc., recognized as a leader in the management and provision of the highest quality community-based services. SAFY s services span across eight states Alabama, Colorado, Indiana, Kentucky, South Carolina, Nevada, Oklahoma, and Ohio. Impact Statement Our service delivery model is a collaborative effort between the family, the Department for Community Based Services, community service providers, and kinship connections the family identifies. Central to the philosophy of treatment evidenced in all SAFY of KY programs is to empower the family and child in service design, treatment planning, and evaluation. Services are designed with the goal of safety, well-being, and permanency for every family and child we serve. SAFY s Model of Care is based on an integrated family systems approach utilizing trauma-informed behavioral health interventions through five primary programmatic services: Family Preservation & Reunification; Therapeutic Foster Care; Behavioral Health Services; Older Youth Services; and Adoption Services. Over the past three years, SAFY of KY has increased the number of youth served in its Independent Living program by 20% each year; currently 30 youth are receiving Independent Living services. Youth exiting the Independent Living program now leave care with a furnished apartment. Additionally, SAFY of KY recognized the number of young mother s exiting foster care and expanded its services to provide parenting support and resources such as cribs. Due to the quality of its program SAFY of KY has become a provider of choice for Independent Living services. 2
3 Needs Statement SAFY of KY engages in this work as a service to youth and their families. The young people we serve face numerous challenges, including higher rates of poverty, homelessness, mental illness and depression, joblessness, and teen pregnancy as well as lower rates of educational attainment. Yet they face their future with hope and resiliency. Ensuring that we have a program that adequately meets their needs and prepares them for financial stability and a healthy productive life requires a highly trained and dedicated work force. It is a continuous challenge to recruit and retain qualified staff to meet the needs of communities in rural areas. CEO/Executive Director Statement The clinical expertise provided through our Model of Care produces positive, sustainable change in the lives of families and their children. Our Older Youth/Independent Living Services promote health development and prepare youth emancipating from the foster care and juvenile justice systems. Highly trained staff help youth mater skills essential for the transition from foster care to adulthood, including safe housing, employment and education. Board Chair Statement We see each family and child we serve as unique, interdependent and adaptable. To us, family is defined not simply by a set of facts, but by a series of circumstances. Not just by one event, but a chain of them. Not by a single symptom, but a set of causes. Each day, we live our enormous Mission of Preserving Families and Securing Futures. When you re helping not just one person, but also a family, you must work holistically. It s a different way of thinking for the social services sector, and it requires a different way of doing, too. Our Model of Care treats every family as a whole because that s how we see the big picture of how to help. Beyond addressing an issue for one, we can address an issue for all. Beyond solving problems for now, we can prevent them from returning in the future. We study causes as clues to systemic problems and challenges that affect and influence the family as a unit. That way, we can provide an inclusive set of solutions and resources to address both the immediate issues and their underlying causes. Working differently requires an awareness that change is needed, the perseverance to be a change agent, and an intentional workforce to make change happen. Our passion is to strengthen families and children through our promise of a progression of care that provides a brighter today and strength for them to carry throughout their lifetime. We know trends in family and child well-being are impacted by many causes, and in failing to support the whole family we can negatively affect a child s ability to live up to their full potential in life. That s why we will remain vigilant in our pursuit of collaborative partnerships and innovative approaches to serve those in our care. That s why we will embrace new technologies to enhance our capacity to ensure sustainable outcomes. That s why there s SAFY. 4 Impact Impact Goals Our goal is to safely maintain children in their natural environment, through a multi-generational approach, which is rooted in Family System Theory. The basis for a family-centered approach is focusing attention on the family, sub-systems within the family and the family's interaction with their community. Strategies We take pride in delivering trauma-informed services. SAFY of Kentucky embraces an organizational mindset about trauma, reflecting adherence to the key principles: Safety, Trustworthiness and Transparency, Peer Support, Collaboration and Mutuality, Empowerment, Voice and Choice, Cultural, Historical and Gender Issues. We work with the National Child Traumatic Stress Network to ensure our staff has current and ongoing trauma informed training and development. Changing the lens in which we plan, resource, implement and provide our services, we examine how we think and respond to those who have been exposed or at risk of exposure to trauma, not only with the families and youth we serve, but with our staff and community members seeking to help and support through: Engaging a family at first contact with a warm and compassionate approach, while stressing the voluntary nature of our services. Adhering to a behavioral management policy which focuses on positive reinforcement with no physical contact. Regularly participate in organization-wide trainings on the most up to date brain development research and complex trauma impact. Demonstrating cultural and linguistic family engagement. Strongly emphasizing staff self-care. Delivering comprehensive staff orientation, peer mentoring, ongoing training, supervision and coaching. In addition to our work in developing a trauma-informed organization, SAFY of KY has spent years of research and study on attachment and the many ramifications of how it plays out in creating a healing home and permanency for children and youth. Research shows us nothing is more effective or important than creating relationships with families who are in crisis. 4
5 Capabilities We are committed to utilizing Evidence-Based Practices whenever possible to address the challenges facing our community. Through education and training, clinical staff have built a toolkit of evidence-based practices to address the unique needs of each family. - Multi-Generational Engagement - Our service continuum for families is centered around family engagement through a multi-generational approach. Delivered 24/7 in a flexible service and crisis support services paradigm, including respite as needed and available. - Multidimensional Family Therapy (MDFT) a SAMSHA recognized Evidence-based Program, offers a recover model that provide participants with treatment and support to achieve sustainable change. Consistent with SAFY's Model of Care, MDFT addresses the individual, family and environmental factors that contribute to substance abuse and related problems. Interventions are collaborative and treatment emphasizes compassion and respect from highly trained clinicians. - Nurturing Parenting curriculum provides a nurturing philosophy and non-violent parenting focuses on the development of empathy, self-worth, self-awareness, empowerment and discipline with dignity, appropriate family roles and age appropriate expectations of children's development. Indicators Through our Organization Development and Research Department (OD&R), we monitor and evaluate the performance, quality and overall effectiveness of our clinical services to families and youth on an ongoing basis. The OD&R develops outcome indicators for SAFY of Kentucky programs and services to evaluate the true impact of the service delivered. Through a Continuous Quality Improvement approach, we identify opportunities to improve efficiencies, our effectiveness and the impact our services have on clients. Improvement plans are created and implemented for all quality improvement opportunities. Progress We conduct client satisfaction feedback on an annual basis. Responses demonstrate families are satisfied with the services provided by SAFY of Kentucky clinical staff. The following results are from 212 responses to the Family Satisfaction Survey July 1, July 1, 2016: - 96% satisfied with our services - 97% satisfied with how they are treated by SAFY - 97% satisfied with their treatment plan involvement -91 5
