2 Mission & Areas Served Statements Mission Statement To educate and assist individuals in the change process needed to live lawfully in society free of substance abuse. Background Statement The Dismas House of Kansas City, Inc.'s was introduced to Kansas City, Missouri in the spring of Our basic mission is to educate and assist men and women to live lawfully in society free of substance abuse. According to the agency's history, the name "Dismas" represents the "good thief" on the cross who in his dying hour asked to be remembered in paradise. The concept was simply it's never too late to reform if an individual so chooses. Dismas House Kansas City, Inc., has an established successful track record of providing effective community based life skills enhancement services to thousands of men and women who at least one time in their lives obstructed the law within the United States. Our geographical reach includes but is not limited to Jackson County and neighboring counties (such as Johnson County and Wyandotte County). In 2017, Dismas House KC will provide more than 150,000 hours of outpatient substance abuse treatment and basic life skills services to more than 3000 men and women who are seeking freedom from addictions and lawless behaviors. At least eighty-one percent of the people served will avoid returning to incarceration for new offenses and sixtyfive percent will abstain from drug use. These outcomes will translate into a savings to tax payers in millions of dollars per year. The savings alone substantiate the need for Dismas services; not to mention the lives that are transformed. Consistently, Dismas House of Kansas City, achieves successful outcomes that exceed national benchmarks. Impact Statement In 2016, Dismas House KC successfully and effectively touched the lives of over 3000 men and women seeking recovery. Dismas House focuses on helping individuals suffering from addiction and/or mental health issues, many of whom have some criminal history in their lives, sometimes brought on specifically by their addiction or mental illness. The majority of these individuals are indigent, under-served and uninsured, and sometimes feel abandoned and hopeless. Through the precious acts of kindness by our Kansas City philanthropic community and funded contract services Dismas House was able to increase its service capacity in Dismas House increased its targeted program outcomes. Approximately 75% abstained from drug use and 80% did not recidivate. We maintained contact with approximately 80% of clients (Life Recovery Members) after they completed their initial treatment and/or recovery services with Dismas House. (In 2011 after reflecting on the type of long term relationship we offer and hope to maintain with each client, our staff redefined those clients as "Life Recovery Members" signifying a lifelong relationship.) We continued this concept and name change in 2012 and it appears to have very positive influence on our relationship with our Life Recovery Members or LRMs. Dismas House will maintain its basic service goals for 2017 for its Life Recovery Members: 1.Reduce or eliminate any drug or alcohol use. 2. Improve LRM's overall legal status in the community so they can enjoy living freely with their families in their homes and communities. 3.Help LRM's secure and/or improve employment status. 4. Help each individual identify with a higher power to lean on through his or her journey of life. We continue to grow and are now faced with having to once again expand our program space to accommodate the people coming to us for help. Needs Statement Each year, Dismas House of Kansas City saves the state of Missouri over $16 million by providing substance abuse treatment and recovery support services to individuals who would otherwise be incarcerated. Most of our consumers have no way to pay for the services they need, leaving Dismas House will nearly $1 million in uncompensated care. Our need support includes program assistance, staff, training, equipment, supplemental services and general operating support. 2

3 CEO/Executive Director/Board Chair Statement The excellent and committed leadership of Dismas House of Kansas City, Inc. consists of CEO Rev. Ladell Flowers, who has served with Dismas House Kansas City since 1972, and COO Sheriece L. Moses, who has been employed with our agency for 9 years. They love Dismas and have a strong belief in their program. The Board of Dismas House Kansas City is a strong nucleus of 7 members who are well respected pillars in our communities with great integrity. Our challenge is to continue to attract talented board members. It is important to secure additional grants and funding for programming to add stability and ability to pay staff. Our current funding from the Department of Mental Health and COMBAT are great sources of support however, the demands for the work we do in our community continues to soar. We need help to sustain our capacity to meet the new demands. Our staff has increased from three in 2006 to 18 as of this writing. 4 Programs Programs Dismas House KC Outpatient Substance Abuse Counseling program Description The Outpatient Substance Abuse Counseling program consists of the following interventions: 1. Individual Counseling 2. Group Counseling 3. Group Education 4. Employment Assistance 5. Drug screens 6. Spiritual Awareness Counseling 7. Faith-Based Recovery Support services 8. Emotional Wellness classes 9. Anger Management Classes1 10. Domestic Violence Classes 11. Recovery Support 12. Parenting classes. We continued our state certified Substance Abuse Traffic Offender Program. Category Mental Health, Substance Abuse Programs, General/other Outpatient Substance Abuse Treatment Population Served Adults,, Short-Term Success Long- Term Success Program Success Monitored By Examples of Program Success Abstinence from substance abuse and unlawful behavior. Clients will disengage in substance abuse and/or felonious behaviors and engage with their families, communities, and employment careers. We use a Logic Model. We follow up on the model outcomes at least annually. We also utilize periodic drug screens and community law enforcement reviews. 100% of our Substance Abuse Traffic Offender Program clients completed their assigned programs. Sixty-five percent of our outpatient clients abstained from drug use and 81% did not recivate back to criminal activity. 4

Client's driving record is monitored by the state for up to ten years. The state Department of Mental Health may contact the Department of Revenue to review the driving record of any SATOP client who has completed the program. President's Faith-based Initiative Program for Recovering Addicts Description Category Population Served Short-Term Success Long- Term Success Program Success Monitored By Examples of Program Success A faith based approach to assist people in discontinuing the use of drugs Mental Health, Substance Abuse Programs, General/other Outpatient Substance Abuse Treatment Adults,, Other Named Groups Client successfully completes the four month program with a clean drug screen as the close of his/her program. Client abstains from substance abuse and re-engages into a faith based or community based recovery support system. Individual drug screens and a six month month follow up after the client completes the program. Sixty-one percent of our clients complete the program free of illegal drugs. CEO Comments In 2017, Dismas House of Kansas City, Inc. will celebrate its 45th year of helping individuals to change the behaviors that once caused them to live a life of criminal behavior and/or drug abuse. Our mission is to assist individuals in a change process needed to live lawfully in society free of substance abuse. Through 40 plus years of faithful service Dismas House has developed a healing and correctional counseling approach to re-entry and substance abuse counseling services that has consistently produced success. National Health and Social Science data statistically confirm that, there are more deaths, illnesses, and disabilities from substance abuse than from any other preventable health condition, that exists in our nation. According to the U.S Department of Labor, the annual cost of on-the-job substance abuse is estimated to be $100 billion. More than half of all child abuse is directly related to drug abuse. National Justice Department figures report that over 60% of criminal arrests relate to illicit drug affiliations in some way. Substance abuse is systematically eating away at the very fiber of the family and societal structure of America. Despite the devastating affect existing in our country concerning substance abuse, (especially as it relates to violence and criminal activity), it can be treated and prevented by changing behavior. Focusing on the five key criteria for influencing and changing behavior, ( Attitude, Commitment, Environment, Resources, and Spirituality,) Dismas house has experienced successful outcomes of offenders and/or substance abusers that 5

6 consistently exceed national benchmarks. Effective outpatient and re-entry services like those provided by Dismas House save taxpayers millions of dollars each year. The average annual incarceration cost is $18,000. Dismas average cost per outpatient services is $1,300. Over half of the 1600 people touched by Dismas House in 2011 received direct treatment and recovery support services. If you do the math, preventing 81% of 800 people from incurring $18,000 for incarceration Tax payers in Missouri saved $14 million alone from Dismas House's efforts. Dismas House completed its first year of its new four-year grant with the Missouri Department of Mental Health's Alcohol and Drug Division. This is a block grant to Missouri funded by the United States Center for Substance Abuse and Treatment. It is community- based and faith-based program. 6

In 2008 Rev. Flowers was selected to serve a 3-year term as Substance Abuse Advisory Council Board member for the Missouri Department of Mental Health Alcohol and Drug Division. The Advisory Council serves as an advisory body to the division (ADA) and the division director on policy, prevention and treatment activities in the state of Missouri. The council was created by Missouri Statute found in section , Missouri Revised Statues. Reverend Flowers' selection to this board demonstrate his the level of recognition he and Dismas House KC in this field of work. In 2010 Reverend Flowers increased his state credentialing to Certified Criminal Justice Professional. This credentialing is reciprocal in 17 state and multiple International communities in the field of substance abuse and correctional counseling. 8 Experience/Biography Sheriece L. Moses, Director of Marketing & Development Sheriece L. Moses brings more than 15 years experience in public relations, advertising and marketing to Dismas House of Kansas City, Inc. With an undergraduate degree from University of Missouri s School of Journalism and a graduate degree from Webster University s School of Business & Technology, Moses has a unique balance of for-profit and servicecentered marketing experience, and has integrated her knowledge of brand strategies, promotions and communications to garner increases in sales, reshape public perception and develop new business opportunities. From the Missouri Department of Health and Truman Medical Centers to clients in the fast-food and retail industries, Moses has been a champion of innovation, growth and community relations. While most recently serving as the vice president of marketing and public relations for the Boys & Girls Clubs of Greater Kansas City, Moses played a valued role in focusing marketing efforts to improve, enhance and retool the process of garnering support. Her volunteer activities include serving on the public relations committee for the Lee s Summit Chamber of Commerce; serving on the marketing committee for the Young Friends of the Symphony and a co-chair of the Young Friends committee for the Kansas City Friends of Alvin Ailey. 9 Document Destruction Policy Collaborations Dismas House maintains collaborations with local, state and federal resources to provide holistic services. We consistently partner with organizations that both provide similar services or provide services to similar audiences; and those organizations whose services provide vital assistance to the network of resources our consumers need. Dismas House has been a vital leader in the creation and advancement of the Kansas City Substance Abuse Treatment and Recovery Supports Coalition. This coalition includes law enforcement, judicial system representatives, social service agencies, health care providers, community-based resources and faith-based affiliations. The coalition has been recognized as a national model for its diversity and ability to collaborate and mobilize. Government Licenses Is your organization licensed by the government? CEO Comments 2011 was a strategic and pivotal year for our agency. It was probably the most significant operating year of the agency since our re-structuring campaign began in 2003 after our transition from a 100 plus bed residential facility to a full outpatient operation. One of the critical goals for the restructuring was to exponentially expand Dismas' community reach in terms of collaborating and promoting our operations and combined spiritual/clinical approach to our work. We became involved in multiple collaborative relationships that continue to exist. Dismas leads a grass root effort form a 15-agency coalition to successfully win a new Four year contract from the Missouri Department of Mental Health to provide substance abuse treatment and recovery support services to people in the Kansas City and surrounding areas who suffer from substance use disorders. In the first year of this grant this new coalition developed the largest ATR III Recovery Support operation in Missouri. This coalition was able to serve more indigent, people suffering from substance abuse issues that any of the other regions in the state rendering ATRIII services. State tracking data showed that this coalition also effectively followed up with 80 percent of its clients served. This coalition and the ATRIII in Kansas City have been led by Dismas House of Kansas City's executive director Reverend Ladell M. Flowers. Reverend Flowers was also recently elected by his peers in Missouri to become the Chairman of the Missouri Department of Mental Health's Alcohol and Drug Division State Advisory Council. The council has a State statutory mandate to advise the Department in such matters as operation policies and evidence based practices in the field for substance abuse. 9

We now know more about effective treatment, recovery and healing for substance abusers and re-entry than ever before in our agency s existence. Focusing on the five key criteria for influencing and changing behavior, (Attitude, Commitment, Environment, Resources, and Spirituality,) we have experienced successful outcomes of offenders and/or substance abusers in our programs that consistently exceed national benchmarks. We have always proposed that our unique professional blend of a faith-based therapeutic community environment with our clinical counseling experience perpetuates our consistent success with our services to our program referrals. In 2008 when we served approximately 150 consumers, approximately 67% of our clients abstained from substance abuse and 88% did not recidivate to incarceration. In 2013, when we served 2,000 consumers for direct services, 78% remained drug free 6-months following treatment and 81% did not recidivate to incarceration.
