2 Statements & Search Criteria Mission Baptist Memorial Health Care Foundation supports the Baptist Memorial Health Care System, which provides excellent care to patients in Tennessee, Arkansas and Mississippi. We partner together to develop the advancement of patient care, addressing current health issues, helping those most in need, and molding new generations of health care professionals. Building on a 103-year tradition of providing award winning care for patients from birth to the end of life, our passion is to raise funds and awareness for projects and services that encourage healthy behaviors and provide excellent health care for each of our patients and families. Our Baptist mission motivates our compassion, and strengthens our ability to reach above and beyond, and to truly transform what it means to be a heath care provider, while honoring our vision of the three-fold ministry of Christ - healing, preaching and teaching. Background Starting from modest beginnings more than 100 years ago, and growing to provide care for a range of Mid- South communities, we have never wavered from our original vision of providing compassionate and quality patient care while instilling a mission of preaching, teaching and healing. Today we have grown to become a health care system of 14 hospitals, 4 minor medical centers, 4 home care programs, 5 hospice programs and one residential hospice house. We are dedicated to providing compassionate, high-quality care for patients. The pervasive spirit of caring inspires every area of operation at Baptist, and throughout our many affiliate hospitals in the Mid-South. 2
3 Impact Each year, Baptist Memorial Health Care Foundation works to raise donations that impact the following areas of need: Taking care of our children- Child and Pediatric Care Build it and they will come has certainly proven true for the Spence and Becky Wilson Baptist Children's Hospital. Opened in January of 2015, the Emergency Department volume has far exceeded expectations. The utilization is indicative of the need to provide quality, accessible emergency and in-patient care to our children. The next phase is to construct a Pediatric Intensive Care Unit for critically ill children. The cost to build the PICU is $4 million. With over 71 percent of our children coming through the Emergency Department insured under Tenn-Care, charitable dollars are critically important due to shrinking reimbursements. Community support will allow us to accelerate the construction of the PICU for our smallest patients and their families. Focus on Parenting - Counseling for Parents In April of 2014 the nation's first two Universal Parenting Places opened in the City of Memphis at Baptist Memorial Hospital for Women and Knowledge Quest. Universal Parenting Places (UPP) are judgmentfree zones where any parent can receive professional counseling, information, and emotional support for familyrelated issues or concerns. Most parenting programs in Memphis, and throughout our nation, target only highrisk or low-income parents, often after problems have developed and carry some stigma. But because research shows toxic stress in families crosses all socio-economic, racial, and ethnic barriers, UPP provides counseling to all parents, regardless of income, and are free of charge. Honoring Aging The Memory Center An estimated 5.3 million Americans of all ages have Alzheimer's disease and of course that number will continue to grow as our population ages. Baptist is pro-active in meeting the needs of Mid-Southerners with the Baptist Memory Care Center, which offers free clinical memory assessment with no physician referral needed. In addition to the screening, individuals who may be suffering from Alzheimer's, dementia, and other memoryrelated issues are assisted in locating programs, facilities, and other community resources that support the needs of the individual and their caregivers. In addition to our resource networking, our staff offers support in dealing with the emotional and practical changes that are occurring in the patient's life. Taking care of those at the end of life - Hospice Care We believe every patient has the right to compassionate, high-quality hospice care, and we provide financial support to more than half our residential hospice patients, with a $30,000+ a month contribution toward that goal. In addition, we raise money, and in-kind donations to offer each hospice patient an end-of-life wish, which we call Priceless Wishes. For many patients with a life-limiting illness, their wishes are quite simple: have a family reunion, enjoy a loved one's birthday, attend a game, go fishing, or celebrate an anniversary. These wishes are often the last thing our patients enjoy doing, which makes these occasions bittersweet. Sometimes the last days can be the best days, and donations make this possible. Putting back the pieces- Grief Counseling Sometimes we need help coping with the sorrow and grief that can come from the loss of someone close to us. Kemmons-Wilson Family Center for Good Grief provides bereavement care free of charge, including grief groups, individual counseling, and the Camp Good Grief programs for children, teens and adults. More than half our clients have suffered a sudden and traumatic loss of a parent or loved one. Our masters-level counselors are specially trained in grief issues. There is no time limit on the number of visits, and the Center is 100 percent dependent upon charitable contributions and must raise approximately $486,000 annually to fund the Center's operations. The grief is real, but so is the healing. Needs From the beginning of life until the end, Baptist works to raise funds for these critical priority areas. In each area, the Foundation strives to raise awareness about the community issues, increase quality of life, and access to resources within the community, and provide charitable dollars to care for patients regardless of their financial circumstances. 5 Programs Programs Kemmons Wilson Center for Good Grief Description The Kemmons Wilson Family Center for Good Grief provides bereavement care free of charge, including grief groups, individual counseling, and the Camp Good Grief programs for children, teens and adults. More than half our clients are children and teens, and many have suffered a sudden and traumatic loss of a parent or loved one. A large number served are from households well below 200% of the federal poverty line. and more than half of our clients are also children. Our masters-level counselors are specially trained in grief issues. There is no time limit on the number of visits, and the Center is 100 percent dependent upon charitable contributions. Budget $0.00 Target Population Strategy Long term Success Short Term Success Children and Youth (0-19 years) Our long term goal for the program is the expansion of our center to a new satellite office in Midtown. This expansion will allow us to serve an increased number of children and will provide residents of that community and surrounding areas easy access to our grief services. It is difficult to measure the impact that bereavement programs make in the lives of children and their families. Outcomes are individualized and based on each child's specific situation. Our counselors encourage the children to grieve in their own unique, personal way and show them that their reactions are normal and natural. Children grieve differently from adults, and even after facing the death of a loved one, society expects them to continue to perform well in school, both academically and socially. Often the feelings of sadness may never go away, and it is important for these children to receive the counseling they need to accept their loss and thrive in their new normal. 5
6 Universal Parenting Place Description Baptist Memorial Hospital for Women and Knowledge Quest, a community nonprofit that works with children in our city's most impoverished neighborhood, have collaborated with the ACE Awareness Foundation to establish the first two pilot Universal Parenting Place (UPP) sites in the nation. UPPs are judgment-free zones where parents receive free professional counseling and personalized information on Adverse Childhood Experiences (ACEs) and toxic stress with long-term support to build skills to cope with the challenges of child-rearing or family-related issues. UPP is designed to curb the effects of, and reduce levels of exposure to ACEs. Working from a family perspective, the UPP centers engage clients in a broad range of interactions, including parent and child activities, counseling (individual, family and group) and a spectrum of other family activities and referrals as appropriate. We designed a solution-based approach to demonstrate how front-end preventative mental health care, not back-end social services, can strengthen the physical, emotional and behavioral health of families. Since opening in 2015, UPP has provided 497 free counseling sessions. Budget $0.00 Target Population Strategy Long term Success Short Term Success Families We cannot expect to eliminate all violence, neglect, abuse and other ACEs from our community, but we can provide caregivers with the tools they need to help reduce the effects of ACEs. We believe that arming parents with these tools and providing them with the support they need will lead to improved kindergarten readiness, decreased ED visits for children, decreased Child Protective Services referrals, and improvements in a myriad of physical health problems for children as they age. We believe that preventative models such as UPP will have positive impacts on our community's children by focusing on the health and wellbeing of the family. Our vision is that within two years, ACE risk questionnaires will be included in initial medical assessments at physician offices, including local primary care physicians, pediatricians and obstetricians. It is also expected that providers will become more knowledgeable about UPP and the services offered, becoming advocates for UPP with their patients and patient families. In the short term, participants will be more competent and confident parents, with decreased stress levels. 6

9 Management Executive Director/CEO Executive Director Jenny Nevels Term Start Nov Experience Jenny Nevels, Executive Director of the Baptist Memorial Health Care Foundation is responsible for all fundraising efforts in support of the charitable mission of Baptist Memorial Health Care. She began her career as Development Officer with the Foundation where she has served for over 20 years, and has been with Baptist Memorial Health Care for a total of 30 years. In 2010 she was promoted to the position of Executive Director. She was instrumental in leading the capital campaign to build the Baptist Reynolds Hospice House, the first residential hospice in our community, and the Kemmons Wilson Family Center for Good Grief raising $12.3 million for this effort. Her efforts in raising funds in support of the area's first bereavement camp for children, Camp Good Grief, led to the creation of the Center for Good Grief, the area's only fully operational bereavement center that provides support to children, teens and adults who have lost loved ones. She was instrumental in establishing the pilot Universal Parenting Place at Baptist Memorial Hospital for Women in collaboration with community based program Knowledge Quest and the ACE Awareness Foundation. The UPP sites provide judgment-free zones where parents can receive information, tools and support to help them with everyday parenting challenges to prevent the exposure of their children to adverse childhood experiences. She is responsible for aligning all fundraising efforts with Baptist Memorial Health Care's strategic plan to enhance patient care, improve quality of care, ensure access to health care for all patients regardless of ability to pay, educate future health care professionals, and support the needs of our community. Jenny is a member of Rotary, a Camp Good Grief and hospice volunteer, and serves as a member of the ACE Executive Committee and Memphis ACE Task Force. She was also selected in 2007 as one of the 50 Women Who Make a Difference.
